摘 要:目的 对青少年近视(低度近视组和中度近视组、低年龄组和高年龄组)角膜塑形术后眼轴变化进行动态分析.方法 临床病例对照研究.选取2009年1月至2012年12月在金华眼科医院配戴角膜塑形镜的青少年63例,男28例,女35例,共121只眼,年龄8~14周岁,球镜度数≤-6.00 D,顺规散光≤-1.50 D,逆规散光≤-0.75 D,通过过夜配戴,观察戴镜后1周、1、3、6、12、18、24个月患者眼轴变化情况,并按近视程度、年龄分为:低度近视组(球镜≤-3.00 D)和中度近视组(-3.25~-6.00 D),低年龄组(8~10岁)和高年龄组(11~14岁).结果 与戴镜前相比,在戴镜6个月后眼轴增长差异有统计学意义(t =4.68,P<0.05).中度近视组和低度近视组眼轴增长在1个月后差异有统计学意义(t=3.4,P <0.05),中度近视组眼轴增长较慢.低年龄组和高年龄组眼轴增长差异无统计学意义(P>0.05).结论 中度近视配戴角膜塑形镜控制近视效果较低度近视佳,而年龄因素对控制近视效果无影响。Objective To analyze dynamic changes of ocular axial length in adolescent myopia (low myopia group and moderate myopia group,low age group and high age group) after orthokeratology.Methods This study elected 63 adolescents of wearing ortho-k in Jinhua Eye Hospital from January 2009 to December 2012,male 28,female 35,a total of 121 eyes,ranging in age from 8 to 14 years old,diopter degree ≤-6.00D,with-the-rule astigmatism ≤-l.50D,against-the-rule astigmatism ≤-0.75D.The changes of ocular axis after 1 week,1 month,3 months,6 months,12 months,18 months,24 months of wearing Ortho-K contact lens through ovemight wear were observed.The volunteers were divided into 4 groups:low myopia group (the Diopter degree ≤-3.00 D) and moderate myopia group (-3.25 ~-6.00 D);low age group (8 to 10 years old) and aged group (11~14 years old).This grouping was based on the degree of myopia and age.Results The growth of ocular axis after six months of wearing Ortho-K was statistically significant compared with unwearing (t =4.68,P 〈0.05).The difference between ocular axial growth of the moderate myopia group and the low myopia group after wearing a month later was statistically significant (t =3.4,P 〈0.05),the axial growth of moderate myopia group was slow,the axial growth between the low age group and the high age group had no significant difference (P 〉0.05).Conclusions The effect of wearing ortho-k contact lens of controlling myopia is better for moderate myopia than low myopia,while the factor of age has no effect on controlling myopia.
