摘 要:目的 观察干性年龄相关性黄斑变性(age-related macular degeneration,AMD)微扫视眼球运动的特点,初步探讨高速眼球运动系统对黄斑疾病视功能评估和量化的价值.方法 前瞻性病例对照研究.收集2013年1~10月在天津市眼科医院就诊的单眼发病的干性AMD患者20例(20只眼)为A1组,对侧眼20只为A2组和视力正常的老年受试者20名主导眼20只眼为N1组,非主导眼20只眼为N2组.采用高速眼球运动记录系统分别对受试者双眼注视性眼球运动进行记录,运用Matlab程序对每组微扫视眼球运动成分进行识别和分析.比较不同组间微扫视幅度与峰值速度之间的差异,及不同组内微扫视幅度与峰值速度、LogMAR视力的相关性.结果 A1组(0.93±0.26)°/微扫视幅度显著高于A2组、N1组和N2组,差异具有统计学意义(F =4.51,P<0.01),A1组(95.00±19.57)°/s峰值速度也较A2组、N1组和N2组显著增加,差异具有统计学意义(F =4.68,P<0.01),相关分析结果显示,A1组微扫视幅度与峰值速度、LogMAR视力均差异有统计学意义(r=0.785、0.579,P<0.01).结论 微扫视幅度、峰值速度等可作为干性AMD患者眼球运动功能客观评价的指标。Objective To observe the effect of dry age-related macular degeneration (AMD) on the behavioral characteristics of human microsaccadic eye-movements.Methods Prospective case control study.From January 2013 to October 2013,20 cases of dry AMD patients (20 dry AMD eyes as group A1 and 20 their sound eyes as group A2) and 20 cases of older adults with normal corrected visual acuities (20 dominant eyes as group N1 and 20 non-dominant eyes as group N2) were recruited from the outpatient clinical Tianjin Eye Hospital.A high-speed eye-movement recording system was used to monocularly record the fixational eye-movements of subjects' both eyes.A Matlab was used to detect and analyze the microsaccadic components of eye-movement waveforms.The microsaccadic amplitudes,peak velocities of all groups were analyzed and compared.The correlation between the microsaccadic amplitudes and peak velocities as well as LogMAR visual acuity within groups was evaluated.Results The mean microsaccadic amplitude of group A1 (0.93±0.26)° was larger than group A2,N1 and N2 (F =4.51,P 〈0.01).The mean peak velocity of group A1 (95.00±19.57)°/s was faster than group A2,N1 and N2 (F =4.68,P 〈0.01).The peak velocities and LogMAR visual acuity were positively correlated with the microsaccadic amplitudes in group A1 (r =0.785,0.579,P 〈0.01).Conclusions Microsaccadic amplitude and peak velocity can be used as the parameters for objective and quantitative evaluation of eye-movement function in dry AMD.
