水平斜视矫正术后眼屈光状态变化

摘　　要：目的 探讨水平斜视矫正术后眼屈光状态的动态变化,以及不同术式对眼屈光状态的影响.方法 前瞻性临床研究.收集2013年9月至2014年4月在青岛大学附属医院眼科行水平斜视矫正术的患者63例75只眼,其中,内斜视17例,外斜视46例进行前瞻性研究.分为外直肌后徙组（Ⅰ组）23例（27只眼）,内直肌后徙组（Ⅱ组）16例（24只眼）,单眼一退一缩组（Ⅲ组）24例（24只眼）.分别于术前1周、术后l周、1、3个月进行屈光检查,统计分析不同术式、不同时间的屈光变化以及手术量与屈光变化的关系.结果 ①球镜屈光度变化：Ⅰ组术后屈光度与术前比较差异均无统计学意义（P＞0.05）.Ⅱ组术后1周、3个月以及Ⅲ组术后l周、1个月屈光度与术前比较差异有统计学意义（P＜0.05）.Ⅰ组与Ⅱ组间术后1周屈光状态变化差异有统计学意义（P＜0.05）.②柱镜屈光度变化：Ⅰ组、Ⅱ组、Ⅲ组术后1周屈光度与术前比较差异均有统计学意义（P＜0.05）,术后1、3个月较术前差异均无统计学意义（P＞0.05）.③手术量与屈光变化关系：手术量与屈光变化程度间未发现明显相关性.结论 不同术式对屈光影响程度及持续时间不同,但均可恢复,手术量与屈光变化程度间无明显相关。Objective To evaluate the dynamic refraction changes following the horizontal strabismus surgery and to study the effect of different surgical procedures on refraction.Methods During September 2013 to April 2014,consecutive patients who underwent horizontal extraocular muscle surgery in the Affiliated Hospital of Qingdao University were prospectively studied.The study included 75 eyes of 63 patients.17 patients were esotropia,and the other 46 cases were exotropia.Twenty-three patients （27 eyes） received lateral rectus muscle recession （group Ⅰ）.Sixteen patients （24 eyes） received media rectus muscle recession （group Ⅱ）.One rectus muscle recession combined with the other resection （group Ⅱ） were performed in 24 patients （24 eyes）.The refractive status of 63 cases （75 eyes） with horizontal strabismus were examined and analyzed respectively at the time of 1 week before surgery and 1 week,1 month,3 months after surgery by optometry.The refraction changes and the correlation between the amount of surgery and the amount of refraction changes were analyzed.Results Sph changes：There was no statistiacal significance Sph changes in group Ⅰ compared with preoperative status （P 〉0.05）.Sph changes which compared with before operation had statistical significance in group Ⅱ at the time of 1 week and 3 months after operation （P 〈0.05）.Patients in group Ⅲ had statistical significance Sph changes at the time of 1 week and 1 month after operation （P 〈0.05）.The difference quantity of the refraction changes of group Ⅰ and Ⅱ at the time of 1 week after operation had statistical significance （P 〈0.05）.Astigmatism changes：Every group had statistical significance changes in astigmatism at 1 week after operation （P 〈0.05）.There was no significance correlation between the amount of surgery and the amount of refraction changes after surgery.Conclusions The amount of refraction changes and the time of duration are different according to different ways of operation
