低度近视性屈光参差儿童眼调节功能临床观察

摘　　要：目的研究低度近视性屈光参差儿童的调节幅度及调节灵活度，比较其与正视儿童调节功能的差别。方法临床病例对照研究。对2012年4月至2013年4月在河北大学附属医院眼科就诊的21例能配合检查的低度近视性屈光参差非弱视儿童（1．0D≤双眼等效球镜差〈2．5D），采用改良移近法及正负球镜翻转法分别测量单眼调节幅度和调节灵活度，以及双眼调节灵活度，并与同期就诊的20例正视儿童作对照比较。结果近视性屈光参差儿童的单眼调节幅度和调节灵活度分别为（16．54±1．33）D和（6．95±2．19）次／min，正视儿童的单眼调节幅度和调节灵活度分别为（16．26±1．97）D和（6．74±1．98）次／min，观察组与对照组的调节幅度和调节灵活度差异无统计学意义（t=1．499、2．276，P=0．028／0．108，P〉0．05）。近视性屈光参差儿童双眼的调节灵活度（4．45±2．09）次／min，正视儿童双眼的调节灵活度（5．95±1．97）次／min，观察组与对照组的差异有统计学意义（t=3．020，P=0．006，P〈0．05）。结论低度近视性屈光参差性儿童双眼调节灵活度明显低于正视儿童，单眼的调节幅度及调节灵活度与同龄正视儿童无明显差别。Objective To investigate the difference of the accommodative amplitude and the accommodative facility between the myopic anisometropic eyes and emmetropic eyes. Methods Twenty-one low myopic anisometropic children were involved, which were assigned with the 1.0D≤spherical equivalent difference 〈2.5D. There were 20 emmetropie children as the control group. The monocular accommodative amplitude was measured with push-up test, and the monocular and binocular accommodative facility was measured with turning over convex and concave lens test. Results The mean value of the monocular accommodative amplitude and the monocular accommodative facility of the myopic anisometropic eyes was （16.54±1.33）D and （6.95±2.19） cycles/min, which was no significant difference from the emmetropic eyes （P 〉0.05）. The mean value of binocular accommodative facility of the myopic anisometropic eyes was （4.45±2.09） cycles/min, which was statistically different from the emmetropic eyes （P 〈0.05）. Conclusions The mean value of binocular accommodative facility of the myopic anisometropic eyes is lower than that of the emmetropic eyes.
