23G微创玻璃体切割系统在2种黏度硅油中应用体会

摘　　要：目的比较1300 centistoke（CS）和5000 centistoke（CS）两种黏度的硅油经23G微创玻璃体切割系统进行硅油注入和取出术的临床效果，探讨1300cs硅油短期填充（〈6个月）在微创玻璃体切割手术中的使用价值。方法临床病例对照研究。回顾性分析自2012年1月至2013年1月在济南市明水眼科医院行23G微创玻璃体切割并硅油注入术患者40例（40只眼），其中1300cs硅油填充眼20例（简称1300cs组），5000cs硅油填充眼20例（简称5000cs组）。随访3～6个月后检查视网膜在位，眼底情况良好，经23G套管针接注射器主动负压吸引装置行硅油取出术。记录并分析两组的硅油注入、取出的时间和并发症情况。结果平均注入量1300CS组为（4．30±0．1298）ml，5000CS组为（4．27±0．1089）ml，两组比较（t=0．924，P=0．361），其差异无统计学意义；平均硅油注入时间1300cs组（42．00±5．0783）s，5000cs组（248．00±32．8361）s，两组比较（t=27．73，P=0．0000），其差异有统计学意义。平均硅油取出时间1300cs组（61．75±7．3261）s，5000cs组（600．70±8．9507）s，两组比较（t=-208．38，P=0．0000），其差异有统计学意义。在硅油填充期间两组患眼均未发现硅油乳化、继发高眼压等并发症，并发白内障发生两组各2例，两组比较（t=0．0000，P=1．0），其差异无统计学意义。结论1300cs硅油较5000cs硅油在23G微创注入及取出时，明显缩短了时间，减少了注入、取出硅油的难度，且短期内（〈6个月）硅油的稳定性好、安全性高。Objective To compare the clinical effect of 23G micro-incision vitrectomy system with two different centistoke silicone oil （1300 cs and 5000 cs） tamponade and removal. The value of using 1300 cs silicone oil in short-term （〈6 months） in micro-incision vitrectomy was observed. Methods A clinical case-control study. Clinical data of 40 patients （40 eyes） who received 23G micro-incision vitrectomy filled with silicone oil were studied retrospectively from January 2012 to January 2013 in Jinan Mingshui Eye Hospital. Among the patients, there were twenty cases filled with 1300 cs silicone oil （1300 cs group for short） and twenty cases filled with 5000 cs silicone oil （5000 cs group for short）. The follow up period was from 3 to 6 months. Funduscopic examination found retinal anatomic reattachment with good eye condition in all patients. Silicone oil was removed under negative pressure using 23G trocar access syringe instrument. The operation times and complications of silicone oil tamponade and removal in two groups were recorded and analyzed. Results Average amount of silicon oil tamponade was 4.30±0.1298ml in 1300 cs group and 4.27±0.1089ml in 5000 cs group. No differences were found between two groups （t =0.924,P =0.361）. Average surgery time of silicon oil tamponade was 42.0015.0783 seconds in 1300 cs group and 248.00±32.8361 seconds in 5000 cs group. A significantly difference was found between two groups （t =-27.73, P =0.0000）. Average surgery time of silicon oil removal was 61.75±7.3261 seconds in 1300 cs group and 600.70±8.950 seconds minutes in 5000 cs group. There were significantly differences between two groups （t =-208.38, P =0.0000）. No complications of silicone oil emulsification and elevated intraocular pressure were found in two groups in the period of silicon oil tamponade. Complicated cataract was found in two patients in each group. Two cases of complicated cataract were found in each group. No differences were found between two groups （t =0.0000, P =1.0）
