紫杉醇联合奈达铂同步放化疗与单纯放疗治疗局部晚期宫颈癌的疗效比较

摘　　要：目的:比较紫杉醇联合奈达铂同步放化疗与单纯放疗治疗局部晚期宫颈癌的疗效。方法:回顾性分析2010年1月至2011年12月来我院进行诊治,经检查和病理等确诊为局部晚期宫颈癌患者82例,根据患者治疗方法分为对照组和观察组,每组41例,对照组进行单纯放疗,观察组使用紫杉醇联合奈达铂同步放化疗,治疗后随访36个月,比较两组患者的近期和远期治疗效果。结果:治疗4个周期后1个月评价两组患者的近期疗效,对照组总有效31例,总有效率为75.61%(31/41),观察组总有效38例,总有效率92.68%(38/41),两组患者总有效率相比,差异具有统计学意义(P<0.05);随访三年中,对照组患者局部复发13例,局部复发率为31.70%(13/41),远处转移9例,远处转移率为21.95%(9/41),3年生存28例,生存率为68.29%(28/41);观察组患者局部复发4例,局部复发率为9.76%(4/41),远处转移2例,远处转移率为4.88%(2/41),3年生存36例,3年生存率为87.80%(36/41),两组患者局部复发率、远处转移率、3年生存率相比,差异具有统计学意义(P<0.05)。结论:紫杉醇联合奈达铂同步放化疗具有较好的近期治疗效果,而且可以提高患者的生存率、降低局部复发率和远处转移率,值得临床上推广应用。Object： To compare the therapeutic effect of Pa- clitaxel combined with Nedaplatin concurrent chemotherapy and radiotherapy in treatment of locally advanced cervical can- cer with radiotherapy. Methods： Retrospectively analyzed 82 patients confirmed locally advanced cervical cancer after exami- nation and pathology from January 2010 to December 2011, di- vided into control group and observation group according to the treatment methods of patients, 41 cases each group, control group treated by radiotherapy, and observation group treated by Paclitaxel combined with Nedaplatin concurrent chemotherapy and radiotherapy, after treatment followed up for 36 months,compared the two groups with short-term and long-term thera- peutic effect. Result： After 4 cycles of therapy 1 month to eval- uate the recent curative effect of two groups of patients, the con- trol group total effectiveness was 31 cases, the total effective rate was 75.61% （31/41）, the observation group total effective- ness was 38 eases, the total effective rate was 92.68%（38/41）, the total efficiency compared in the two groups, the difference had statistical significance（P〈0.05）; 3 years of following up, the local recurrence patients in the control group were 13 cases, the local recurrence rate was 31.70% （13/41）, distant metastasis were 9 cases, the distant metastasis rate was 21.95%（9/41）, and 3 years survival were 28 cases, the 3 years survival rate was 68.29%（28/41）; the local recurrence patients in the observation group were 4 cases, the local recurrence rate was 9.76%（4/41）, distant metastasis were 2 cases, the distant metastasis rate was 4.88% （2/41）, and 3 years survival were 36 cases, the 3 years survival rate was 87.80%（36/41）, the local recurrence rate, dis- tant metastasis rate and 3 years survival rate between the two groups compared, the difference had statistical significance（P〈 0.05）.
