鼻咽未分化癌患者LKB1表达及其在放疗中应用价值

摘　　要：目的探讨鼻咽未分化癌患者肿瘤组织中LKB1蛋白表达情况及其在放疗中应用的临床意义。方法采用免疫组织化学SP法检测65例鼻咽未分化癌患者肿瘤组织中LKB1蛋白表达情况,并依据检查结果将其分为阳性组和阴性组,记录2组年龄、性别比例、EB病毒阳性、远处转移情况,放疗结束后4周评定2组疗效,并随访观察2组3a生存率。结果 65例中鼻咽未分化癌组织LKB1阳性39例(阳性组),LKB1阴性26例(阴性组);阳性组颈部淋巴结转移率(38.46%)低于阴性组(69.23%)(P<0.05),2组年龄、性别比例、EB病毒阳性表达率比较差异无统计学意义(P>0.05);阳性组总有效率(71.79%)高于阴性组(46.15%)(P<0.05),3a生存率(58.97%)高于阴性组(26.92%)(P<0.05)。结论 LKB1蛋白与鼻咽未分化癌颈部淋巴结转移有关,LKB1阳性患者常规放疗疗效及远期预后优于LKB1阴性患者。Objective To explore the expression of LKBI and its clinical significance in the radiotherapy of undifferentiated nasopharyngeal carcinoma. Methods The expression of LKB1 protein was detected in 65 patients with undifferentiated nasopharyngeal carcinoma by SP immunohistochemical method, and the patients were divided into positive group and negative group according to the expression of LKB1. The parameters of age, sex, positive rate of EB virus and distant metastasis were recorded in both groups. Four weeks after the end of radiotherapy, all patients received the therapeutic evaluation and the 3-year survival was observed in two groups. Results In 65 patients, LKB1 was positive in 39 （positive group） and negative in 26 （negative group）. The distant metastasis rate was significantly lower in positive group （38.46%） than that in negative group （69.23%） （P〈0.05）. The effective rate and 3-year survival rate were higher in positive group （71.79%, 58.97%） than those in negative group （46.15%, 26.92%） （P〈0.05）. There were no significant differences in the age, sex and the expression rate of EB virus between two groups （P〉0.05）. Conclusion LKB1 protein is correlated with cervical lymph node metastasis in patients with undifferentiated nasopharyngeal carcinoma. The patients with positive LKB1 expression have a better therapeutic effect and long-term prognosis than those with negative LKB1 expression after conventional radiotherapy.
