摘  要:目的探讨创伤性失血性休克(traumatic hemorrhagic shock,THS)大鼠血浆可溶性细胞间粘附因子-1(soluble intercellular adhesion molecule-1,sICAM-1)的动态变化及其临床价值。方法选择无特定病原体成年大鼠17只,通过急性失血方式建立收缩压下降曲线,形成THS动物模型,测定收缩压基础值(T1),记录收缩压降至基础值3/4(T2)、1/2(T3)、2/5(T4)、1/5(T5),以及10mmHg(T6)的失血量,采用酶联免疫吸附方法检测各个时间点血浆sICAM-1表达。结果T5~T6时间段组内差异有统计学意义(P〈0.05),T2~T5时间段组内的差异无统计学意义(P〉0.05),T5~T6时间段组内sICAM-1表达水平明显升高。结论选择收缩压下降曲线建立THS模型评估失血量变化,结合血浆sICAM-1的动态监测,可对血管内皮细胞损伤程度与THS相关过程进行有效评估,sICAM-1可作为判断血管内皮细胞炎性损伤反应的血清学指标。Objective To explore the dynamic change of soluble intercellular adhesion molecule-1(sICAM-1)in sera of rats with traumatic hemorrhagic shock(THS).Methods The THS model of 17 rats was established with decrease of systolic pressure by acute hemorrhag.The base line of systolic pressure(T1)was tested.The blood loss volumes of the rats were recorded when the pressure was declined to three-forth(T2),one-second(T3),two-fifth(T4),one-fifth(T5),and 10 mm Hg(T6)of the base line.ELISA was used to examine the sICAM-1levels at various time points.Results There was a significant difference in serum level of sICAM-1between the T5 and T6(P〈0.05),and no striking difference was noted between the T2 and T5(P〉0.05)The sICAM-1level in T5-T6 was significantly increased.Conclusion The rat model of THS for decline curve of systoloc pressure in shock is helpful for assessment of blood loss.The dynamic levels of seral sICAM-1,a useful marker of vascular endothelial cells injury is recommendable for assessment of the THS and its related pathology of the vascular endothelial cells.
