摘 要:目的探讨非小细胞肺癌(NSCLC)恶性胸腔积液中ERCC1、XRCC1表达水平与铂类药物化疗疗效及预后关系。方法收集2012年3月—2014年3月给予铂类药物方案治疗的伴有恶性胸腔积液的晚期NSCLC患者82例,采用流式细胞术检测胸水标本中ERCC1、XRCC1蛋白表达水平,Kaplan-Meier生存曲线分析比较不同ERCC1和XRCC1表达水平患者的中位生存期(OS)和无病进展时间(PFS)。结果 ERCC1的阳性率为41.5%(34/82),XRCC1的阳性率为34.1%(28/82)。ERCC1阴性表达者含铂方案的化疗效果优于阳性表达者,但两者间比较差异无统计学意义(χ^20.63,P=0.43);XRCC1阴性表达者含铂方案的化疗效果优于阳性表达者,两者间比较差异亦无统计学意义(χ^20.01,P=0.93)。ERCC1和XRCC1阴性表达的患者OS明显长于阳性表达者(χ^25.91,P〈0.05),PFS亦明显长于阳性表达者(χ^215.31,P〈0.05)。结论胸腔积液中肿瘤细胞ERCC1、XRCC1表达与晚期非小细胞肺癌的预后具有相关性,与铂剂化疗疗效的关系尚待进一步明确。Objective To investigate the relationship between ERCC1, XRCC1 expression level in malignant pleural effusion of non-small cell lung cancer (NSCLC) and the efficacy of platinum based chemotherapy and prognosis, Methods From 2012 March to 2014 March, 82 Cases receiving platinum drug regimens for the treatment of malignant pleural effusion of advanced NSCLC were enrolled, using flow cytometry to test ERCC1, XRCC1 protein expression level in pleural effusion, using Kaplan-Meier survival curve analysis to compare different ERCC1 and XRCC1 expression level in patients with median survival (OS) and stage to disease free progression (PFS). Results The positive rate of ERCC1 was 41.5% (34/82), the positive rate of XRCC1 was 34.1% (28/82). ERCC1 negative expression with platinum based chemotherapy was more effective than the positive expression of persons, but no significant difference between the two regime ( χ^2 = O. 63, P = 0.43) ; XRCC1 negative expression contain platinum based chemotherapy was more effective than the positive expression, no signifi- cant difference were found between the two regime (χ^2 = 0.01, P = 0.93 ). The negative expression of ERCC1 and XRCC1 patients' OS was significantly longer than the positive expression patients (χ^2 =5.91, P 〈0.05) ; PFS also was significantly longer than that of the positive expression patients ( χ^2 = 15.31, P 〈 0.05 ). Conclusion Pleural effusion ' s tumor cells of ERCCI, XRCC1 expression is related to the prognosis of late stage small cell lung cancer, and the relationship with platinum agent chemotherapy remains unclear.
