TWIST和雌激素受体α在乳腺癌组织中的表达及对预后的影响

摘　　要：目的探讨转录因子TWIST和雌激素受体(ER)α在乳腺癌组织中的表达及对预后的影响。方法选取2014年1月-2015年1月河南省人民医院病理科存档的70例乳腺浸润性小叶癌患者临床资料,采用免疫组织化学法和RT-PCR检测乳腺癌和癌旁组织的TWIST和ERα的表达,分析患者的临床病理特点、乳腺癌组织TWIST和ERα表达相关性以及不同TWIST和ERα表达水平乳腺癌患者的生存率及复发率。结果乳腺癌组织的TWIST表达阳性率(77.1%)明显高于癌旁组织(8.5%)(P<0.05),乳腺癌组织的ERα表达阳性率(32.9%)明显低于癌旁组织(74.1%)(P<0.05);乳腺癌组织的TWIST的m RNA表达量明显高于癌旁组织,ERαm RNA表达量明显低于癌旁组织(均P<0.05);TWIST和ERα在乳腺癌组织中的表达呈负相关(r=-0.68,P=0.008);TWIST和ERα的表达与乳腺癌患者的病理分级和淋巴转移有相关性(P<0.05);TWIST表达阳性和ERα表达阴性的乳腺癌患者的5年期复发率明显高于其他组(均P<0.05),5年期生存率明显低于其他组(均P<0.05);病理分期、淋巴转移、TWIST以及TWIST(+)/ERα(-)是影响乳腺癌生存预后的因素。结论 TWIST呈高表达的乳腺浸润性小叶癌组织中ERα呈低表达,TWIST和ERα的表达呈负相关,TWIST高表达以及ERα低表达状态不利于乳腺癌患者的预后,值得临床诊断和治疗中加以关注。【Objective】To investigate the expression of the transcription factor TWIST and estrogen receptor（ER）α in breast cancer and its effect on prognosis. 【Methods】From January 2014 to January 2015, 70 patients with invasive lobular carcinoma in Department of Pathology in our hospital were chosen to study. The expression of TWIST and ERα in breast cancer and adjacent tissues was detected by immunohistochemical method and RT-PCR. The clinical and pathological features of the patients and the correlation of TWIST with ERα were analyzed, and the survival and recurrence rates in the breast cancer patients with different TWIST and ERα expression were also determined. 【Results】The positive rate of TWIST expression in the breast cancer（77.1%） was significantly higher than that in the adjacent tissues（8.5%）（P 0.05）. The positive rate of ERα expression in the breast cancer（32.9%） was significantly lower than that in the adjacent tissues（74.1 %, P 0.05）. The expression level of TWIST m RNA of the breast cancer was significantly higher than that of the adjacent tissues, ERα m RNA expression in the breast cancer was significantly lower than that in the adjacent normal tissues（all P 0.05）. There was a negative correlation be-tween TWIST and ERα expression in the breast cancer（r =-0.68, P = 0.008）. TWIST and ERα expression in the breast cancer patients was correlated with pathological grade and lymph node metastasis（P 0.05）. The 5-year recurrence rate of the breast cancer with positive TWIST expression and negative ERα expression was significantly higher than that of other groups（all P 0.05）, the 5-year survival rate of the breast cancer with positive TWIST expression and negative ERα expression was significantly lower than that of other groups（all P 0.05）. Pathologic stage, lymph node metastasis, TWIST and TWIST（＋）/ERα（-） were the prognostic factors in breast cancer survival.【Conclusions】Invasive lobular carcinoma tissues with high TWIST expression have
