三联药物治疗与牙周基础治疗对幽门螺杆菌清除率的影响

摘　　要：目的探讨三联药物治疗与牙周基础治疗对幽门螺杆菌(Hp)清除率的影响。方法抽选93例经胃镜诊断为消化性溃疡、活检尿素酶试验阳性且合并有慢性牙周炎的患者,采用随机分层法分对照组(例数=45例,予以三联药物治疗)和观察组(例数=48例,在三联药物治疗的同时还予以了牙周基础治疗),治疗结束后分别于3、6个月采用聚合酶链式反应法(PCR)检测患者口腔及胃部Hp清除率,观察观察组牙周临床指数变化。结果观察组患者牙周治疗后的菌斑指数(PLI)、探诊深度(PD)、出血指数(BI)与治疗前比较明显好转(P<0.05)。观察组患者3、6个月的口腔Hp清除率均明显高于对照组(P<0.05);胃部Hp清除率至6个月时亦显著高于对照组(P<0.05)。结论对消化性溃疡合并有慢性牙周炎的患者予以三联药物治疗的同时予以口腔专科牙周基础治疗,能显著提高胃内Hp远期根除率。Objective To investigate the effect of triple drug treatment and periodontal treatment on the clearance of Helicobacter pylori. Methods Totally 93 cases of patients with peptic ulcer diagnosed by endoscopy, biopsy urease test positive with chronic periodontitis were selected and randomly divided into control group （n = 45 cases, triple drug treatment） and the observation group （n = 48 cases, in a triple drug treatment at the same time to periodontal therapy）, After treatment respectively for 3, 6 months, polymerase chain reaction（PCR） was used to detect the clearance rate of patients with oral, gastric Hp, the clinical periodontal index of patients in the observation group were observed. Results Compared with be- fore treatment, the plaque index（ PLI）, probing depth（PD）, bleeding index（BI） in the observation group after treatment were significantly improved（P 〈0. 05）. In the observation group 3, 6 months of oral clearance rate of Hp were significantly higher than those in control group（P 〈 0. 05） ; Stomach Hp scavenging rate to 6 months were significantly higher than that of control group （P 〈 0. 05）. Conclusion Patients with peptic ulcer complicated with chronic periodontitis should be treated by the triple drug and periodontal, it can significantly improve the intragastric Hp longterm eradication rate.
