枳术汤治疗非糜烂性反流病及对5-羟色胺影响

摘　　要：目的:观察枳术汤治疗非糜烂性反流病(NERD)的疗效及其对食管下端黏膜5-羟色胺(5-HT)阳性细胞的影响。方法:以埃索美拉唑为对照,予20例NERD患者8周枳术汤。观察治疗前后反流症状积分、食管下括约肌张力(LESP)、食管远端收缩波幅、De Meester评分、反流次数的变化。免疫组化检测5-HT阳性细胞及其平均光密度(OD值)的变化。结果:两组治疗前后反流症状积分、De Meester评分均减少(P<0.05);中医治疗组LESP、食管远端收缩波幅治疗后较治疗前升高(P<0.05),对照组两项指标无明显变化(P>0.05);两组治疗前后反流次数、OD值均减少(P<0.05),且两组间均数有差异(P<0.05)。结论:枳术汤对NERD患者有一定疗效,可能与其增加LESP、增加食管远端收缩波幅、减少反流次数及调节5-HT代谢有关。Objective： To observe the effect of Zhizhu Decoction on non-erosive reflux disease （ NERD ） and the serotonin （ 5-HT ） positive cells of distal esophagus mucosa. Methods ： Compare to esomeprazole, the change of Zhizhu Decoction on reflux symptoms integral, LESP, contraction amplitude of the distal esophagus, DeMeester score, numbe^r of reflux, the serotonin （ 5 - HT ） positive cells was investigated in 20 patients of NERD after 8 week＇s treatment. Results ： Reflux symptoms integral, DeMeester score, number of reflux, OD score of the 5 - HT positive cells was reduced by Zhizhu Decoction and esomeprazole （ P〈0.05 ）. LESP, contraction amplitude of the distal esophagus, was increased by Zhishu Decoction （ P〈0.05 ）, but not esomeprazole （ P〉0.05 ）. Conclusion ： The good effect of Zhizhu Decoction on non-erosive reflux disease may be related to the increase of LESP, contraction amplitude of the distal esophagus, and the decrease of number of reflux and the 5 - HT positive cells.
