12例替牙期大型颌骨囊性病变开窗减压治疗的疗效评价

摘　　要：目的：研究开窗减压治疗术对儿童替牙期颌骨囊性病变的效果,评价其在功能外科的运用价值和对颌骨、牙胚生长发育的影响。方法：回顾分析12例采用开窗减压术治疗的儿童替牙期囊性病变病例,随访1~9 a,术后患者每3个月接受临床和影像学复查,观察和分析囊腔的大小变化和牙胚萌出情况。结果 ：替牙期颌骨大型囊性病变经开窗减压术后明显缩小、颌骨生长发育及牙胚的发育未受影响,长期观察未见复发。结论：开窗减压术是儿童替牙期颌骨囊性病变的有效治疗方法,其主要优点是手术方法简单,经济、安全、效果良好,且颌骨、牙胚生长发育不受影响。部分位置异常牙可自行调整位置,部分病例则需要结合正畸治疗。PURPOSE： To assess the outcome of large jaw cysts in mixed dentitions treated with decompression, and explore the value of surgery in preservation of function and its impact on growth and development of jaw and dental germ.METHODS： Twelve cases were treated with decompression and followed up for 1-9 years. The patients underwent clinical and radiological examinations every 3 months, the size of the cyst, eruption of the dental germ were measured and recorded. RESULTS： After decompression, the giant jaw cysts became smaller obviously, the growth and development of jaw were not disturbed. The development of tooth germ was also not affected. Long term observation showed no recurrence.CONCLUSIONS： Decompression is one of the effective ways to treat giant jaw cysts in mixed dentitions. Its advantage is simple, economic, safe, effective without adverse effect on jaw and teeth development. Normal alignmentt of permanent teeth can be achieved in some patients, while some needs further orthodontic treatment.
