盐酸丙美卡因联合0.5%利多卡因在超声乳化白内障摘除联合小梁切除手术中的应用

摘　　要：目的探讨在超声乳化白内障摘除联合小梁切除手术中,表面麻醉剂盐酸丙美卡因和0.5%利多卡因联合应用的临床效果。方法 78例(78只眼)行超声乳化白内障摘除联合小梁切除手术中,35只眼采用单纯表面麻醉盐酸丙美卡因进行麻醉;43只眼采用盐酸丙美卡因联合0.5%利多卡因进行麻醉。记录手术前后的矫正视力\眼压以及术中术后并发症。结果术后眼压单用盐酸丙美卡因组比联合应用利多卡因组略有减少,但是两组差异无统计意义;单用盐酸丙美卡因组和联合应用利多卡因组术后视力提高者分别28例(80%)和34例(79.1%),无视力下降及丧失者,差异无统计学意义。单用盐酸丙美卡因组术中及术后并发症者13例(37.1%);联合应用利多卡因组术中及术后并发症者10例(23.3%),其中角膜上皮水肿差异有统计学意义,其他并发症差异无统计学意义。结论在超声乳化白内障摘除联合小梁切除手术中联合应用表面麻醉剂有助于手术的安全、顺利实施,并可减少手术并发症,值得推广应用。Objective To investigate the effects of proparacaine hydrochloride and 0.5% lidocaine anesthesia in phacoemulsification and Trabeculectomy surgery.Methods In 78 patients who received phacoemulsification and trabecu-lectomy, one of two anesthesia methods were given.Simple surface anesthesia with proparacaine hydrochloride alone was given to 35 patients, and other 43 patients were given proparacaine with hydrochloric acid 0.5%lidocaine.Corrected visu-al acuity before and after surgery, intraocular pressure （ IOP） and intraoperative complications were recorded and compared between these two groups.Results IOP was slightly lower in patients who were given lidocaine but this difference was not statistically significant.In patients who were given hydrochloric acid alone, visual acuity improved in 80%of the cases （ n=28）, while the rate was 79.5%（ n =34） in those who were given lidocaine jointly.There was no difference between the groups.There was no decrease in visual acuity in any patient.Intraoperative complications occurred in 13 patients （37.1%） who received hydrochloric acid alone, and in only 10 patients （23.3%） who received lidocaine jointly.Among all the complications, the rate of corneal edema was significantly different between the groups.Conclusion Topical anes-thesia with proparacaine hydrochloride and 0.5% lidocaine jointly may help to smooth the IOL implementation and to re-duce surgical complications.
