不同时期糖尿病患者白内障超声乳化吸除术后黄斑厚度比较

摘　　要：目的观察不同时期糖尿病患者白内障术后黄斑厚度的改变。方法对30例年龄相关性白内障患者、30例轻中度非增生期糖尿病视网膜病变(NPDR)患者及30例重度非增生期及增生期糖尿病视网膜病变(PDR)患者白内障术前及术后1个月行黄斑区神经上皮层厚度检查,并分析。结果轻中度NPDR患者术前黄斑中心凹平均厚度(250.32±21.34)μm,术后1个月平均厚度(261.12±23.52)μm,重度NPDR及PDR患者白内障术前黄斑中心凹平均厚度(253.36±21.58)μm,术后(280.65±30.48)μm,年龄相关性白内障患者术前黄斑中心凹厚度(245.45±19.50)μm,术后(255.10±22.54)μm。结论重度NPDR及PRD患者较轻度NPDR患者白内障术后黄斑区厚度明显增加,黄斑水肿发生率高。Objective To study the foveal thickness following cataract surgery in patients with various stages of di-abetic retinopathy.Methods Cataract patients were divided into three groups according to their diabetic retinopathy sta-ges：mild-to-moderate non-proliferative （ group A） , severe non-proliferative or proliferative （ group B） , and control （ with-out diabetic retinopathy;group C）.Each group consist 30 patients.Foveal thickness was measured by 3D-OCT before sur-gery and on the 30th day after surgery.Results The foveal thickness was 250.32 ±21.34 μm before surgery, and in-creased to 261.12 ±23.52 μm 1month after the surgery in group A.The pre-and postoperative thickness in group B was 253.36 ±21.58 μm and 280.65 ±30.48μm.In control group, the pre-and postoperative thickness was 245.45 ±19.50μm and 255.10 ±22.54 μm.Conclusion There is a higher incidence of macular edema after cataract surgery in patients with diabetic retinopathy, especially in those who have severe non-proliferative diabetic retinopathy or proliferative diabetic retinopathy.
