摘　　要：目钓探讨T1～2期原发性直肠癌患者淋巴结转移的相关因素，为更好地确定直肠癌局部切除的适应人群奠定科学基础。方法选取在本院肿瘤外科住院行根治性手术的T1～2期的原发性直肠癌且治疗的临床资料完整的患者82例，将患者的性别、年龄、肿瘤的直径、肿瘤的组织类型、肿瘤的大体类型、分化程度、浸润深度、T分期以及术前癌胚抗原（CEA）水平等指标进行回顾性分析。结果肿瘤的组织类型、肿瘤的大体类型、分化程度、浸润深度、T分期为T1～2期原发性直肠癌患者淋巴结转移的相关影响因素；将上述原因纳入Logistic回归多因素分析中，结果表明肿瘤的分化程度、T分期为T1～2期原发性直肠癌患者淋巴结转移的独立影响因素。结论肿瘤的分化程度与T分期是T1～2期原发性直肠癌患者淋巴结转移的独立影响因素，对于分化程度较高Tl的患者可以考虑行局部切除术。Objective To investigate the related factors of the T1-2 primary patients with lymph node metastasis of rectal cancer, in order to better establish the scientific basis for defining suitable crowd for local excision of eolorectal cancer. Methods 82 T1-2 patients with primary coloreetal cancer and with complete clinical data in Surgical Oneology of the hospital underwent radical surgery, gender, age, tumor diameter, tumor histological type, tumor gross type, degree of differentiation, depth of invasion, T stage, preoperative earcinoembryonic antigen （CEA） levels and other indicators were analyzed retrospectively. Results The tumor tissue types, tumor gross type, degree of differentiation, depth of invasion, T staging were related factors in patients with primary colorectal cancer lymph node metastasis of T1-2; The Logistic regression multivariate analysis included the above reasons, the results showed tumor differentiation, T stage were independent predictors of lymph node metastasis in patients with primary colorectal cancer stage T1-2. Conclusions The degree of tumor differentiation and T stage were independent predictors of lymph node metastasis in patients-with primary eolorectal cancer T1-2 period, for a higher degree of differentiation may be considered in patients with T1 local excision.
