儿童肾上腺神经母细胞瘤的超声诊断价值

摘　　要：目的探讨超声检查诊断儿童肾上腺神经母细胞瘤的临床应用价值及超声图像特征。方法回顾性分析28例经穿刺活检或术后病理证实为肾上腺神经母细胞瘤患儿的超声图像特征表现。结果 28例肾上腺神经母细胞瘤体位于一侧肾上腺区,右侧15例,左侧13例;瘤体最大径线平均(8.3±1.2)cm,21例形态不规整,24例瘤体内出现多发沙砾样或斑片状、点条状强回声钙化灶,9例瘤体内伴液化无回声,22例瘤体内血流丰富,12例包绕腹部大血管,瘤体较大者占位效应明显,出现肾积水7例;20例患儿治疗后行超声随访:15例化疗后病灶体积缩小,瘤体内强回声钙化灶密集、增多,局部液化无回声增多,13例术后瘤体残留,复发3例,转移5例。结论儿童肾上腺神经母细胞瘤的超声检查图像表现具有一定特征性,其在儿童肾上腺神经母细胞瘤的诊断及鉴别诊断、治疗后随访等方面具有重要的临床价值。Objective To assess the ultrasonic characteristics of pediatric adrenal neuroblastoma. Methods The sonogram features of 28 cases of adrenal neuroblastoma confirmed by needle biopsy or postoperative pathology were analyzed. Results Tumors were located on one side of the adrenal area. The maximum diameter was （8.3 ± 1.2） cm. Twenty-one tumors had irregular shapes, 24 tumors had multiple gravel-like, patchy or point/bar calcifications, 9 tumors appeared cystic liquefaction echo,22 tumors had rich blood flow signals and 12 tumors were wrapped around abdominal large vessels. Larger tumors had obvious mass effect with hydronephrosis in 7 cases. Twenty cases were followed-up after treatment. In 15 cases, the ultrasonography after chemotherapy showed that tumors were smaller with more concentrated calcification and more anechoic liquefaction. Postoperative ultrasonic findings showed tumor residues in 13 cases, recurrence in 3 cases and metastatic lesions in 5 cases. Conclusion The ultrasonic appearance of pediatric adrenal neuroblastoma has certain characteristics, and ultrasonographic examinations are of clinical value in diagnosis and follow-up after treatment.
