摘 要:目的:探讨能谱 CT 成像在肺腺癌射频消融联合生物治疗疗效评价中的应用价值。方法收集本院经病理证实的肺腺癌患者34例,行射频消融治疗联合生物治疗,治疗前和治疗1-3个月后应用能谱 CT 增强扫描,在70 keV 单能量下测量瘤体相同实性成分的碘、水含量及 CT 值。测量瘤体最大径按 RECIST 标准分组评估疗效:缓解(CR)、部分缓解(PR)、病情稳定(SD)、病情进展(PD),对治疗前后各组数据做自身配对 t 检验。结果34例患者中 CR 5例、PR 19例、SD 7例、PD 3例,有效率(CR+PR)为70.59%,疾病控制率(DCR)为91.18%。70 keV 单能量下治疗前碘含量为(12.88±3.42)x100μg/cm3、水含量为(1029.24±8.87)mg/cm3、CT 值为(59.38±9.48)HU,治疗后碘含量为(5.64±1.60)x100μg/cm3、水含量为(1028.14±8.76)mg/cm3、CT值为(38.63±11.18)HU。治疗后碘含量和 CT 值较治疗前均降低,差别有统计学意义(P 〈0.01)。结论射频消融治疗联合生物治疗对肺腺癌有较好的疗效,能谱 CT 成像技术能够对射频消融联合生物治疗的肺腺癌患者的疗效进行很好的评估。Objective To study the spectral CT imaging application in the evaluation of curative effect of radiofrequency ablation combined with biological treatment in lung adenocarcinoma.Methods Thirty-four patients with lung adenocarcinoma confirmed by histopathology were treated by radiofrequency ablation combined with biological treatment in our hospital.Spectral CT enhanced scan was used to before and 1-3 months after the treatment.The contents of iodine,water and CT values of the same solid parts of the tumors were measured at the 70 keV level.And the largest diameter of tumors were also measured.The effect evaluation were divided according to RECIST criteria:complete response (CR),partial response (PR),stable diseases (SD),and progressive diseases (PD).The groups of data before and after the treatment were compared by matching t test.Results Among the 34 cases,5 cases were CR,19 cases in PR,7 cases in SD,3 cases in PD.The effective rate (CR+PR)was 70.59%.The disease control rate was 91.18%.The iodine con-tent before treatment was (12.88±3.42)x100 μg/cm3 ,water content was (1 029.24±8.87)mg/cm3 ,CT value was (59.38±9.48)HU at the 70 keV level.And after the treatment the results were (5.64 ±1.60)x100 μg/cm3 ,(1 028.14 ± 8.76)mg/cm3 and (38.63±1 1.18)HU, respectively .The iodine content and CT value after treatment was lower than before .The different between the two groups was statistically significant(P 〈 0.01).Conclusion Radiofrequency ablation combined with biological treatment is a good curative method for lung adenocarcinoma,and spectral CT imaging technology could be very useful in the evaluation of the effects.
