摘 要:目的评估MRI T2WI与弥散加权成像(DWI)预测垂体腺瘤质地的价值,分析肿瘤质地与胶原含量的关系。方法分析38例垂体腺瘤的术前MRI,分别在T2WI、表观弥散系数(ADC)上测量肿瘤与脑桥的信号强度比(RT2和RADC),并与肿瘤胶原含量进行比较。结果肿瘤质软13例,质中17例,质韧8例。不同质地的垂体腺瘤之间,在T2WI信号值、RT2、ADC、RADC之间均有差异(均P<0.05)。RT2与肿瘤质地一致性最高。质软组垂体腺瘤平均胶原含量为(1.51±0.91)%,质中组为(7.35±2.99)%,质韧组为(18.10±8.24)%,三组有明显差异(P<0.05)。结论 T2WI和ADC均有预测垂体腺瘤质地的价值,以T2WI更可靠。垂体腺瘤质地越韧,胶原含量越高。Objective To estimated the value of T2WI and diffusion weighted imaging (DWI) in detecting the texture of pituitary adenoma, and study the relation between texture and collagen content. Methods The preoperative imagings of 38 patients with pituitary adenomas were analyzed respectively, including the ratio of signal intensity of tumor to ports in TzWI and DWI (R12 and RADC), and then compared with the collagen content of pituitary adenoma. Results The tumor consistency at surgery was classified as soft in 13 patients, intermediate in 17 and hard in 8. The signal intensity (SI) differences between pituitary adenomas with different textures on TEW1 (T2PA, RT2, ADC and RADC were statistically significant (all P 〈 0.05). There was a highest degree of concordance between the RT2 and tumor texture. The mean collagen content was 1.51% ± 0.91% in the soft, 7.35 % ±2.99 % in the intermediate and 18.10 % ± 8.24 % in the hard texture group, there were statistic differences between the three groups (P 〈 0.05). Conclusions TzWI and ADC are of a certain value in forecasting the consistency of tumor, and the TEWI is more reliable. The harder the pituitary texture is, the higher the collagen content.
