血清HE_4与CA_(125)对卵巢上皮性癌诊断价值及其与临床病理特征的关系研究

摘　　要：目的：探讨血清人附睾蛋白4（HE4）与癌抗原125（CA125）检测对卵巢上皮性癌的诊断价值及其与临床病理特征的关系。方法：测定47例卵巢上皮性癌患者和48例卵巢良性疾病患者血清HE4和CA125水平,通过HE4与CA125的检查值计算出二者诊断卵巢上皮性癌的敏感性、特异性,并绘制ROC曲线,计算ROC曲线下面积（AUC）,分析比较两者的诊断效能。并分析血清HE4与CA125水平与卵巢上皮性癌临床分期及病理组织分级的关系。结果：1卵巢上皮性癌组血清HE4、CA125平均水平（中位数分别为534.53 pmol/L和687.26 U/ml）均显著高于卵巢良性疾病对照组（中位数分别为46.65 pmol/L和32.86 U/ml,P〈0.01）。2HE4单项检测的AUC为0.952（95%CI 0.910-0.995）,临床诊断效能优于CA125（AUC 0.852,95%CI 0.772-0.952）。3在卵巢上皮性癌患者中血清HE4的表达水平随患者临床分期、病理组织分级的增高而上升;血清CA125的表达水平随临床分期、病理组织分级的增高而上升不明显。结论：血清HE4具有比CA125更优的诊断卵巢上皮性癌的价值。血清HE4的表达水平随患者临床分期、病理组织分级的增高而上升较CA125明显,可作为卵巢上皮性癌筛查的肿瘤标志物。Objective： To investigate the value of human epididymis protein 4（ HE4） and cancer antigen（ CA125） in diagnostic values and pathological features of ovarian epithelial cancer. Methods： The level of HE4 and CA125were measured in the serum specimens of 47 cases of ovarian epithelial cancer（ ovarian epithelial cancer group）,48 cases of benign ovarian disease（ control group）. Sensitivity,specificity and ROC curve and the area under the ROC curve were calculated to explore the diagnostic effciency of HE4 and CA125measurements for ovarian cancer. Moreover the relationship of HE4 and CA125level in clinical stages and pathological features of ovarian cancer were analyzed. Results： 1The median levels of HE4 and CA125（ 534. 53 pmol/L vs. 687. 26 U/ml） of ovarian epithelial cancer group were significantly higher than those of the benign ovarian diseases group（ 46. 65 pmol / L vs.32. 86 U / ml,P〈0. 01）; 2The area under the ROC curve（ AUC） of HE4 was lager（ 0. 952,95% CI 0. 910 - 0. 995） than that of CA125（ 0. 852,95% CI 0. 772-0. 952）; 3 In ovarian epithelial cancer group,expression level of HE4 was significantly increased with clinical stages and pathological staging incresed,however,expression level of CA125 was not positively related to clinical stages and pathological staging significantly. Conclusions： As an ovarian epithelial cancer marker,HE4 is better than CA125. The positive relationship between expression level of HE4 and clinical stages and pathological staging is more significant than CA125,therefore HE4 can be used as tumor marker for ovarian epithelial cancer screening.
