摘 要:目的:研究环氧合酶-2(COX-2)在宫颈癌组织中的表达及对预后的影响。方法选择2008年6月~2009年10月在邯郸市第一医院及邯郸市中心医院住院治疗的原发宫颈癌患者的蜡块共95例,采用免疫组化S-P法检测宫颈癌中COX-2的表达情况,并探讨其与宫颈癌预后的关系。结果 COX-2在95例宫颈癌组织中的阳性表达率为65.26%(62/95),不同淋巴结转移、肌层或脉管浸润情况及肌层浸润深度的宫颈癌组织中阳性表达率比较,差异均有统计学意义(P〈0.05)。相关性分析显示院COX-2与肿瘤直径、淋巴结转移及宫旁或脉管浸润密切相关(r=0.207、0.220、0.215,均P〈0.05)。 Log-rank分析发现院COX-2、临床分期、组织学分级、肿瘤直径、淋巴结转移及宫旁或脉管浸润与宫颈癌生存率有关(P〈0.05)。多因素分析发现院COX-2与临床分期、淋巴结转移、肿瘤直径、宫旁或脉管浸润及肌层浸润深度均为宫颈癌不良预后的独立危险因素。结论 COX-2为宫颈癌预后的危险因素,对判断预后及指导治疗具有重要的临床意义。Objective To study the expression of COX-2 in cervical malignant epithelial tissue and the effect on the prognosis of cervical cancer. Methods 95 paraffin block of primary cervical cancer in Handan Municipal First Hospital and Handan Municipal Central Hospital from June 2008 to October 2009 was selected. The expression of COX-2 was identified by immunohistochemically S-P method in 95 paraffin section, and the relationship on the expression of COX-2 and the prognosis of cervical cancer was discussed. Results The positive expression rate of COX-2 was 65.26%(62/95) in 95 patients with cervical cancer, the positive expression rate of COX-2 in cervical cancer with different situ-ation of lymphnode metastasis, latero-uterus or vessel infiltration and infiltrative depth of muscle had statistically sig-nificant differences (P〈 0.05). The expression of COX-2 was positively correlated with tumor diameter, lymph node metastasis, latero-uterus or vessel infiltration (r=0.207, 0.220, 0.215, all P=0.037). Log-rank test showed that COX-2, clinical stage, histological grades, tumor diameter, lymph node metastasis and latero-uterus or vessel infiltration were related with survival rate of cervical cancer (P〈0.05);multiariable analysis showed that COX-2, clinical stage, tumor diameter, lymph node metastasis, latero-uterus or vessel infiltration and infiltrative depth of muscle were risk factors af-fecting the survival of patients with cervical cancer. Conclusion COX-2 is factor that effect on prognostic of cervical cancer, which could be served as a prognostic parameter and direct clinical therapy of cervical cancer.
