胃部分切除手术治疗早期胃癌的疗效观察

摘　　要：目的：探讨和研究胃部分切除术治疗早期胃癌的临床疗效。方法研究对象选取为2012年1月～2013年12月我院收治的54例早期胃癌患者，根据手术方案不同分为观察组和对照组各27例，观察组采用胃部分切除术治疗，对照组则采用远端胃切除术治疗，对比两组患者的胃排空和胆囊收缩功能情况，并统计两组患者的近期复发率。结果术后测定结果显示，观察组患者各时点的胃排空状态均优于对照组，差异具有统计学意义（P＜0.05），口服乙酰氨基酚45 min、60 min后观察组的胆囊收缩率优于对照组，差异具有统计学意义（P＜0.05）；随访1年，观察组术后无一例出现复发，复发率0.0%，对照组出现3例复发，复发率11.1%，观察组低于对照组，差异具有统计学意义（P＜0.05）。结论胃部分切除术能够很大程度上保留患者的消化功能，胃排空及胆囊收缩功能均要优于进行远端胃切除治疗，且近期复发率较低，疗效确切。Objective To explore the clinical effect of partial gastric resection in the treatment of early gastric cancer.Methods Fifty cases of early gastric cancer in our hospital were selected as the research object between Jan 2012 to Dec 2013,according to different operation scheme, they were divided into observation group and control group,each group of 27 cases,the observation group with partial gastrectomy in treatment and control group was used for the treatment of distal gastrectomy,compared two groups of patients with gastric emptying and gallbladder contraction function situation and statistics the short-term recurrence rate of two groups of patients.ResultsThe results showed that the gastric emptying of the observation group was better than the control group at each time point,the difference was statistically significant（P〈 0.05）,after oral acetaminophen 45min and 60min,the gallbladder contraction rate of the observation group was better than that of the control group,the difference was statistically significant（P〈 0.05）,following 1 year,no recurrence occurred in the observation group,the recurrence rate was 0%,the control group had 3 cases of recurrence,the recurrence rate was 11.1%,the observation group was lower than the control group,the difference was statistically significant（P〈 0.05）.ConclusionPartial gastric resection can largely retain the digestive function of the patients,gastric emptying and gallbladder contractile function were superior to the distal stomach resection and recent recurrence rate is low,curative effect is exact.
