巨大胸膜孤立性纤维瘤的MSCT表现(附2例病例报告及文献回顾)

摘　　要：目的探讨巨大胸膜孤立性纤维瘤(SFTP)的MSCT表现特点,以提高对本病的诊断率,并指导临床手术治疗。方法搜集经手术及病理证实的2例巨大SFTP患者资料,术前均行MSCT平扫及增强扫描,并行MPR、MIP、VR及CTA等图像后处理,分析总结其影像学表现特点。结果巨大SFTP为肺外胸腔内单发肿块,包膜完整,有蒂,可见分叶,于胸腔内铸型生长,密度欠均匀,血供丰富,呈地图状强化及延迟强化,常见肿瘤血管,供血动脉明确。结论 MSCT可清晰显示SFTP的大小、位置、形态、强化方式及与周围组织的关系,其增强表现具有一定特征性;MSCTA可清楚显示肿瘤血供情况及供血动脉,对制定合理的手术治疗方式具有重要意义。Objective To analyze the CT imaging features of giant solitary fibrous tumor of the pleura（ SFTP）,in order to increase the diagnostic accuracy and to guide surgical treatment. Methods Two patients with surgery and pathology proved SFTP from October 2011 to May 2013 were retrospectively reviewed in this study. They both received plain and contrast enhanced chest CT scans,and three dimensional reconstruction including MPR,MIP,VR and CTA were performed before operation. Results Giant SFTP is extrapulmonary and intrathoracic solitary inhomogeneous intensity mass with well- defined margins and smooth contours. It is well capsuled,pedunculated,lobulated,growing like casting mould in the chest. Tumor shows rich blood supply,＂geographic pattern＂and delayed enhancement pattern. Tumor vessels and feeding arteries can be showed clearly in CTA. Conclusion MSCT can clearly show the size,location,morphology,enhancement pattern of the SFTP and its relationship with the surrounding tissue. Enhancement pattern has some characteristic features. MSCTA is useful for demonstrating the tumor vascularity and feeding arteries,which are important to surgery treatment.
