摘　　要：目的提出并探讨优先阻断肾上腺中央静脉及机械切除肾上腺肿瘤术式的临床价值。方法对2006年1月至2012年10月间南京军区南京总医院采用后腹腔镜优先阻断肾上腺中央静脉并机械下切除肾上腺肿瘤的88例患者的临床资料进行回顾性分析。结果全部手术均顺利完成。平均手术时间(28±15)min,平均术中出血(21±11)ml。恢复饮食和下床活动时间分别为1.2 d和1.0d。全部病例未出现并发症,临床症状消失。结论优先阻断肾上腺中央静脉,并机械切除肾上腺肿瘤组织,出血少,视野清晰,并可避免电凝、超声刀等导致的热损伤,可最大限度地保留正常的肾上腺组织与维护其生理功能,也有利于腺体创面的愈合和临床的早期康复。Objective To investigate the clinical value of priority blocking adrenal central vein and mechanical resection of adrenal tumor surgery. Methods From January 2006 to December 2012, adrenal central veins were blocked and mechanical resection of adrenal tumor surgeries were done in 88 cases of retroperitoneal laparoscopic surgeries. Results All operations were successfully completed. The average operation time was （28±15） rain, the average intraoperative blood loss was （21 ±11）ml. Recovery of diet and ambulation time were 1.2 d and 1.0 d. No complications occurred, clinical symptoms all disappeared. Conclusion Priority blocking adrenal central vein has the best effect of bleeding control during operation, and mechanical resection of adrenal tumors is helpful for the early clinical rehabilitation for glands wound healing with less bleeding, clear vision, meanwhile it can avoid electric coagulation, thermal damage caused by electric coagulation and ultrasonic knife. These can retain the maximum normal adrenal tissue and maintain their physiological function.
