活血化瘀方联合电针治疗脑卒中后肩手综合征疗效观察

摘　　要：目的:观察活血化瘀方联合电针治疗脑卒中后肩手综合征的临床效果。方法:将80例患者分为2组,均以脑卒中后常规疗法治疗,观察组41例采用活血化瘀方联合电针疗法治疗,对照组39例给予康复疗法。采用疼痛视觉模拟评分(VAS)、肩关节活动度(ROM)、Fugl-Meyer肩关节活动评分评定2组患者治疗前后的疼痛程度及关节功能变化。比较2组的临床疗效。结果:治疗后,2组VAS评分、Fugl-Meyer评分与ROM均显著变化,组内比较,差异有统计学意义(P<0.05),且观察组VAS评分低于对照组(P<0.05),Fugl-Meyer评分、ROM均高于对照组(P<0.05)。治疗总有效率观察组为92.68%,对照组为64.10%,2组比较,差异有统计学意义(P<0.01)。结论:活血化瘀方联合电针疗法不仅能减轻脑卒中后肩手综合征患者的肩部疼痛程度,改善患侧上肢运动功能,而且在扩大肩关节外展和上举活动范围方面有明显疗效。Objective:To investigate the effect of Huoxue Huayu fang combined with electroacupuncture in the treatment of shoulde-hand syndrome after stroke. Methods:Eighty patients with shoulder-hand syndrome after stroke were divided into the observation group(41 cases) which was given Huoxue Huayu fang com bined with electroacupuncture,and the control group(39 cases) which was given rehabilitation therapy. The therapeutic effects,and the scores of Visual A nalog Scale(V A S),range of motion(R O M) and Fugl-Meyer upper limb motor function assessment(FM A) were observed. Results:The scores of V A S and FM A, R O M of both groups were obviously superior to those before treatment(P < 0.05). A nd the difference between the two groups after treatment was significant(P < 0.05). The scores of V A S of the observation group were obviously lower than those of the control group(P < 0.05). The scores of FMA and ROM of the observation group were obviously higher than those of the control group(P < 0.05). The total effective rate was 92.68% in the treatment group,and was 64.10% in the control group, the difference being significant(P<0.05). Conclusion:Huoxue Huayu fang com bined with electroacupuncture can not only relieve the pain of patients with shoulder-hand syndrome after stroke,but also prom ote the ability of movement and range of motion.
