摘 要:目的评价多排螺旋CT灌注成像(MSCTP)在甲状腺癌诊断及鉴别诊断中的应用价值。方法选取甲状腺良性病变患者18例作为良性组,甲状腺癌患者22例作为恶性组。2组患者均进行MSCTP检查,测量并分析比较2组中病变组织与正常组织的血流量(BF)、血容量(BV)、表面通透性(PS)及峰值时间(TTP)灌注参数值。结果良性组病变组织与正常组织的BF、BV、PS和TTP的M值差值较小,病变组织/正常组织的比值较小,差异无统计学意义(P>0.05)。恶性组病变组织与正常组织的BF、BV和PS的M值差值较大,病变组织BF、BV和PS的M值明显低于正常组织,病变组织/正常组织的比值较大,差异有统计学意义(P<0.001);而TTP的M值差值较小,病变组织/正常组织的比值较小,差异无统计学意义(P>0.05)。结论甲状腺癌的病变组织与正常组织的MSCT检查指标中PBF、BV和PS的M值差值及病变组织/正常组织的比值在甲状腺癌的诊断及鉴别诊断中具有重要的应用价值。Objective It is to evaluate the values of using MSCT perfusion imaging in the diagnosis and differential diagno -sis of thyroid carcinoma .Methods 40 cases of thyroid neoplasm were divided into two groups:18 cases of benign tumors as benign group , and 22 cases of malignant thyroid tumors as malignant group .The patients in both groups were scanned with MSCT perfusion, their blood flow (BF), blood volume(BV), permeability surface (PS)and peak time were measured, com-pared and statistically analyzed .Results The mean values of the parameters of BF , BV, PS and TTP in lesion tissues and nor-mal tissues of the group with benign tumors were smaller , the ratio of lesion tissues/normal tissues was small , and the differ-ences were not significant (P〉0.05).The mean values of the parameters of BF , BV, PS in lesion tissues and normal tissues of malignant group were bigger .M value of BF, BV and PS in lesion tissue were obviously lower than that in normal tissue , the ratio of lesion tissues/normal tissues were bigger, the differences were significant(P〈0.001), but M value of TTP was smal-ler, the ratio of lesion tissues/normal tissues was smaller, and the difference was not significant (P〉0.05).Conclusion With CT perfusion imaging , the difference and ratio of BF ,BV and PS between the lesion tissues and the normal tissues play an important clinical role in the diagnosis and differential diagnosis of thyroid carcinoma .
