培美曲塞联合顺铂治疗非小细胞肺癌的疗效研究

摘　　要：目的探讨培美曲塞联合顺铂治疗非小细胞肺癌的疗效。方法选择2012年1月~2014年12月我院住院治疗的非小细胞肺癌患者72例,所有患者均经病理学诊断,其中腺癌32例,鳞癌28例,其他12例。全部患者随机分为治疗组（培美曲塞联合顺铂治疗）和对照组（吉西他滨联合顺铂治疗）,每组各36例,比较两组治疗4个周期后的总有效率、疾病空置率、毒副反应情况。结果治疗组总有效率为41.67%,对照组总有效率为38.89%,两组疗效比较,差异无统计学意义（P〉0.05）;治疗组疾病的总控制率达94.44%,2例病灶增大≥25%,对照组疾病的总控制率达83.33%,5例病灶增大≥25%,1例出现新的病灶。治疗组疾病的总控制率显著高于对照组,差异有统计学意义（P〈0.05）。治疗组出现白细胞减少17例、血红蛋白减少16例、血小板减少14例、恶心呕吐9例、便秘6例、脱发9例、肝肾功能异常2例,对照组出现白细胞减少19例、血红蛋白减少15例、血小板减少13例、恶心呕吐19例、便秘16例、脱发7例、肝肾功能异常3例。治疗组出现恶心呕吐、便秘比率显著低于对照组,差异有统计学意义（P〈0.05）。但两组在骨髓抑制以及其他毒副反应方面比较,差异无统计学意义（P〉0.05）。结论培美曲塞联合顺铂治疗非小细胞肺癌疗效较好,毒副作用少,安全性好,值得广泛推广和应用。Objective To investigate the efficacy of pemetrexed combined with cisplatin in the treatment of non small cell lung cancer. Methods 72 patients with non small cell lung cancer from 2012 January to December 2014 in our hospital were collected, all patients werediagnosed by pathology, including 32 cases of adenoearcinoma, 28 cases of squamous cell carcinoma, other 12 cases. All the patients were divided into treatment group （pemetrexed plus cis- platin） and control group （gemcitabine plus cisplatin treatment）, 36 cases in each group, after 4 cycles of treatment, the total effective rate of the disease, the vacancy rate, side reaction were compared between two groups. Results The total effective rate of treatment group was 41.67%, total effective rate in control group was 38.89%, the difference was not statistically significant （P〉0.05）, total control of disease rate in the treatment group was 94.44%, 2 cases were in- creased more than 25%, the total control of disease rate was 83.33%, the lesions of 5 cases increase more than 25%, 1 cases of the emergence of new lesions. The treatment groupthe total control rate of disease was significantly higher than the control group, the difference was statistically significant （P〈0.05）. The treatment group 17 cases with leukopenia, hemoglobindeereased in 16 cases, 14 cases of thrombocytopenia, 9 cases of nausea and vomiting in 6 cases, 9 cases with hair loss, abnormal function of the liver and kidney in 2 eases, the controlgroup 19 cases with leukopenia, hemoglobin decreased in 15 cases, 13 cases of thrombocytopenia, nausea and vomiting in 19 cases, 16 patients with hair loss, constipation in 7 cases, 3 cases of abnormal liver function. Nausea and vomiting, constipation, the ratio of the treatment group was significantly lower than the control group, the difference was significant（P〈0.05）.
