献血者献血前后血细胞数量变化的研究

摘　　要：目的探讨不同献血量的献血者献血后相关生理指标变化及献血者体内储备池的释放与恢复时间。方法随机选择不同年龄段的自愿无偿献血者63名,分为200和400 m L献血组,于献血前、献血后即刻、献血后1 h、献血后1 d、献血后1周分别采集其血样做血常规检测。结果 63名献血者献血前后各点的血常规数值均在正常范围内;献血前和献血后即刻、献血后即刻和献血后1 h、献血后即刻和献血后1 d以及献血后即刻和献血后1周的红细胞和血红蛋白值的变化明显(P<0.05);200和400 m L组的献血者红细胞和血红蛋白值在献血后1 h可恢复到>93%献血前水平。结论由于机体血细胞储备池的释放,献血者献血后血细胞数量迅速上升,并在献血后1 h达到高峰,在献血后1周自身血液明显得到补充,故捐献400或200 m L全血对献血者外周血细胞数量的影响无明显差别,献血是安全的。Objective To investigate donors＇ related physiological change, the release and recovery time of reserve pool after blood donation by observing the variation in blood cell count at different time before and after various amount of blood donation. Methods 63 volunteer donors of different ages were chosen received routine blood tests before donation, immediately, 1 hour, 1 day and 1 week after donation, and they were divided into 200 mL group and 400 mL group. Results All the data collected at different times were in the normal range. The variation of the blood ceils count was obvious between before and immediately after donation, between immediately after donation and 1 hour, 1 day and 1 week after donation （P 〈 0. 05）. The red blood cell and hemoglobin of two groups at 1 hour after donation can be restored to more than 93% of the original count before donation. Conclusion The blood cell count decreases as the donors lose the blood after donation and tissue fluid replenished blood volume instantly, but they increases immediately and reaches peak at 1 hour after donation as the reserve pool releases the blood cells. The blood volume has been replenished in 1 week after donation. No notable differ- ence on the blood cell counts can be observed after donation between 200 mL donation and 400 mL donation, which explains that blood donation is a safe practice.
