复发鼻咽癌的预后因素分析

摘　　要：目的探讨影响复发鼻咽癌患者预后的相关因素。方法回顾性分析2002-11-06—2011—05-06广州医科大学附属肿瘤医院，经病理或影像诊断确诊的101例复发鼻咽癌患者的临床及随访资料。87例患者接受放疗，61例接受1～6个周期以铂类为基础的化疗。Kaplan-Meier法计算生存率并行Log-rank检验，Cox回归模型进行多因素分析。结果101例复发鼻咽癌患者，中位随访时间26个月（1～117个月）。1年总生存率为73．3％，3年为41．5％。单因素分析结果显示，年龄（χ2=4．656，P=0．031）、原发T分期（χ2=5．494，P=0．019）和复发T分期（χ2=4．301，P=0．038）与复发鼻咽癌患者生存期有关；Cox模型多因素分析结果显示，原发T分期是影响复发鼻咽癌患者生存时间的独立影响因素，χ2=4．843，P=0．028。结论年龄、原发T分期及复发T分期对复发鼻咽癌患者的生存期均有影响，而原发T分期是影响患者生存的独立预后因素。OBJECTIVE To analyze the prognostic factors of recurrent nasopharyngeal carcinoma （rNPC）. METHODS Clinical and follow-up data of 101 patients diagnosed with rNPC by pathology and/or imaging were collected in the Affiliated Cancer Hospital of Guangzhou Medical University from November 6,2002 to May 6,2011. Eighty-seven patients were treated with re-irradiation and sixty-one patients with 1--6 courses of cisplatin-based chemotherapy. Survival analysis was estimated using Kaplan-Meier method and compared by the Log-rank test. Multivariate analysis was performed using Cox proportional hazards model. RESULTS The median follow-up time was 26 months （ranged from 1 to 117 months）. Overall survival rate of 1- and 3-year was 73.3% and 41.5% ,respectively. The result of univariate analysis showed that age （χ2 =4. 656,P=0. 031） ,primary T stage （ χ2= 5. 494,P=0. 019） and recurrent T stage （χ2= 4. 301, P= 0. 038） were positively correlated with the survival time. Multivariate analysis showed that primary T stage was an independent risk factor for rNPC （χ2= 4. 843,P= 0. 028）. CONCLUSIONS Age,primary T stage and recurrent T stage are positively correlated with the survival time. Primary T stage is an independent risk factor for patients with recurrent nasopharyngeal carcinoma.
