摘　　要：目的:探讨原发性前上纵隔肿瘤的临床特点、诊断方法、外科治疗。方法:回顾性分析和研究经外科手术治疗的71例原发性前上纵隔肿瘤患者的临床资料。结果:71例患者均经手术治疗,术后病理证实为原发性前上纵隔肿瘤。71例患者中,60例良性肿瘤,11例恶性肿瘤,全组均完整或大部分切除,手术均顺利完成。术后出现并发症5例,无明显严重并发症。合并肌无力患者在术后6个月内临床症状基本消失,其他并发症患者持续药物治疗后症状得以明显缓解,其余患者均恢复出院。结论:该病诊断主要依靠胸部X线片、CT等影像学检查;诊断一经成立,应尽早手术治疗。关键在于早期诊断、正确的手术切口和方式的选择以及适宜的围手术期护理。Objective. To investigate the clinical features, diagnosis and surgical treatment of the pri mary anterior superior mediastinum tumors. Methods. Clinical data of 71 patients with primary an terior superior mediastinum tumors having been underwent surgeries were retrospectively analyzed. Results： All 71 patients with anterior superior mediastinum tumors were surgically treated and were pathologically confirmed with anterior superior mediastinum tumors. Of the 71 cases, there were 60 cases of benign tumor and 11 cases of malignant tumor. All patients were partially and completely removed, and the operations were successful. 5 cases appeared postoperative complications, but no obvious serious complications. Clinical symptoms of patients with myasthenia almost disappeared within 6 months after operation~ symptoms of other patients with complications are re lieved after the drug treatment, and the remaining patients recovered and discharged. Conclusions： Chest X-ray and CT examinations are the common access to diagnose anterior superior mediastinum tumors. Once the diagnosis of this tumor is established, surgical treatment should be performed as soon as possible. The key is the early diagnosis, selection of the correct operation incision and mode, and the proper preoperative nursing care.
