摘 要:目的:研究2型糖尿病相关性牙周炎患者龈下菌群含量及其与龈沟液中细胞因子含量的关系。方法:选择2012年3月-2014年8月就诊的2型糖尿病相关性牙周炎患者和单纯牙周炎患者进行研究,分别纳入糖尿病伴牙周炎组和单纯牙周炎组。检测龈下菌群含量以及龈沟液中细胞因子含量。结果:(1)龈下菌群:与单纯牙周炎组比较,糖尿病伴牙周炎组患者的牙龈卟啉单胞菌、伴放线菌嗜血菌、福赛斯坦纳菌、中间普雷沃菌、齿垢密螺旋体含量较高(P〈0.05);(2)抗氧化物质和氧化产物:与单纯牙周炎组比较,糖尿病伴牙周炎组患者的MPO、CAT、SOD、GSH、VitC的含量较低(P〈0.05),MDA、8-OhdG的含量较高(P〈0.05);(3)信号分子:与单纯牙周炎组比较,糖尿病伴牙周炎组患者的SFRP1、Fas、FasL、Wnt5a、NF-kb、p38的含量较高(P〈0.05)。结论:2型糖尿病相关性牙周炎患者龈下菌群中致病菌含量明显增多,且抗氧化物质大量消耗、氧化产物大量产生,伴有多种信号通路功能异常。Objective:To study correlation between subgingival microflora contents and gingival crevicular fluid cytokine levels of type 2diabetes patients with periodontitis.Methods:Type 2diabetes patients with periodontitis and simple periodontitis patients treated in our hospital from March 2012 to August 2014 were selected for study and enrolled in diabetes with periodontitis group and simple periodontitis group,respectively.Then subgingival microflora contents and levels of cytokines in gingival crevicular fluid were detected.Results:(1)Subgingival microflora:compared with simple periodontitis group,contents of porphyromonas gingivalis aeromonas,actinomycetes with haemophilus,Forsyth Tanner bacteria,middle Prairie Waugh bacteria and Treponema denticola of diabetes with periodontitis group were significantly higher(P〈0.05);(2)Antioxidants and oxidation products:compared with simple periodontitis group,MPO,CAT,SOD,GSH and VitC contents of diabetic with periodontitis group were significantly lower(P〈0.05);MDA and 8-OhdG contents were significantly higher(P〈0.05);(3)Signaling molecules:compared with simple periodontitis group,SFRP1,Fas,FasL,Wnt5 a,NF-kb and p38 contents of diabetic with periodontitis group were significantly higher(P〈0.05).Conclusion:Contents of pathogens in subgingival microflora of type 2diabetes patients with periodontitis significantly are increased.Antioxidants are extensively consumed and oxidation products are largely generated.Besides there is dysfunction of multiple signaling pathways.
