管状胃成形重建食管在胸段食管癌切除术中的应用价值分析

摘　　要：目的探讨管状胃成形重建食管在胸段食管癌切除术中的应用价值。方法选择2013年5月~2014年5月间120例食管癌切除术患者,按随机数字表法分为观察组（n=60,行管状胃成形重建食管）及对照组（n=60,行传统全胃代食管吻合术）。对比两组近期疗效,术后3个月及12个月的心肺功能、生活质量。结果观察组术后并发症发生率明显低于对照组（P〈0.05）;观察组术后3个月时生活质量差异无统计学意义（P〉0.05）,但至12个月,观察组生活质量指标均明显高于对照组（P〈0.05）;观察组术后3个月、12个月心肺功能明显高于对照组（P〈0.05）。结论管状胃成形重建食管可明显降低胸段食管癌切除患者术后并发症,提高及改善其远期心肺功能及生活质量,值得推广。Objective To explore the application value of reconstructing esophagus by forming gastric tube in resection of thoracic esophageal carcinoma. Methods All 120 patients with resection of esophageal carcinoma admitted into our hospital from May 2013 to May 2014 were divided into two groups by random number table method. The observation group（n=60） was received esophageal reconstruction by forming gastric tube, while the control group（n=60） was treated with traditional reconstruction of esophagus by whole stomach anastomosis. The recent efficacy, cardiopulmonary func- tion and quality of life 3 months and 12 months after operation of the two groups were compared. Results The occur- rence rate of postoperative complications in the observation group was significantly better than that of the control group （P〈0.O5）; there was no significant difference in quality of life of the observation group 3 months after operation （P〉 0.05）. But 12 months later, quality of life indexes in the observation group were significantly higher than those of the control group（P〈0.05）; 3 months and 12 months later, the cardiopulmonary function of the observation group was signif- icantly higher than that of the control group（P〈0.05）. Conclusion Reconstructing esophagus by forming gastric tube can significantly reduce postoperative complications in patients with resection of thoracic esophageal carcinoma and improve the long-term cardiopulmonary function and quality of life, worthy of promotion.
