牙周干预对2型糖尿病患者脂联素浓度、C-反应蛋白和胰岛素抵抗指数的影响

摘　　要：目的探讨牙周干预对2型糖尿病患者的C-反应蛋白（CRP）、脂联素（APN）浓度、胰岛素抵抗指数（HOMAIR）的影响。方法选择2013年12月~2014年11月间126例伴糖尿病牙周炎患者,根据是否进行牙周干预将其随机分为观察组（n=63）接受牙周干预治疗和对照组（n=63）不接受牙周干预治疗。比较两组患者治疗前后的FPG、Hb A1c、CRP、APN、FINS,计算HOMA-IR。结果两组患者治疗前上述指标比较差异无统计学意义（P〉0.05）,治疗2个月后,两组患者APN较治疗前升高（P〈0.05）,CRP、HOMA-IR均较治疗前下降（P〈0.05）,空腹血糖和随机血糖均有改善,且观察组改善程度明显高于对照组（P〈0.05）。结论牙周干预对2型糖尿病患者的脂联素浓度、C-反应蛋白和胰岛素抵抗具有改善作用。Objective To investigate the impact on adiponectin （APN） concentration, C-reactive protein （CRP）, insulin resistance index （HOMA-IR） in patients with type 2 diabetes by periodontal intervention. Methods All 126 patients with diabetic were selected from December 2013 to November 2014, and they were randomly divided into observation group （n=63） and control group（n=63）, trail paitents were treated with periodontal intervention, and control group were not treated with periodontal intervention. The FPG, HbAlc, CRP, APN, FINS, HOMA-IR of the two groups before and after treatment were compared. Results The serum APN, CRP, HOMA-IR of two groups of patients before treatment, the difference was not statistically significant difference （P〉0.05）; After two months of treatment, APN levels of both groups of patients were increased compared with before treatment, the CRP, HOMA-IR levels were decreased com- pared with before treatment, the differences were statistically significant（P〈0.05）; After treatment, APN, CRP, HOMA- IR levels improved more significantly, the difference was statistically significant（P〈0.05）. After treatment, fasting and random blood glucose were improved, but the improvement of the patients in the observation group was better than the control group of patients, the difference was statistically significant（P〈O.05）. Conclusion Periodontal intervention play an important role in type 2 diabetes adiponectin genotype different C- reactive protein and insulin resistance improvement.
