老年患者卒中后抑郁的危险因素分析

摘　　要：目的探讨老年卒中患者卒中后抑郁的危险因素,并提出可行的护理方法。方法选取2012年6月至2014年12月就诊与我院的老年脑卒中患者190例,采用问卷调查的方式收集患者信息并从患者的病例中寻找患者的疾病信息,对以上收集的各种因素做单因素分析,并对单因素分析出的显著因素做多因素分析,得出老年患者卒中后抑郁的危险因素。结果年龄大于65岁、女性、病程大于1年、病变部位在小脑、脑干、额叶三处、神经功能缺陷程度评分过高为老年患者卒中后抑郁发生的显著因素;患者的性别、年龄、病变部位、NDS是老年患者卒中后抑郁发生的危险因素。结论女性,年龄过高,病变部位为小脑、脑干、额叶三处,NDS高于正常水平是老年患者卒中后抑郁的独立危险因素,对于这部分患者临床医护人员应当给予特殊注意并采取额外的护理措施,以降低其卒中后抑郁的发生率。Objective Analysis the factors causing stroke depression in elderly patients with cerebral stroke, to explore the risk factors of depression in elderly patients after stroke, and put forward feasible nursing method. Method 190 elderly stroke patients from 2012 June to 2014 December in our hospital treatment and the patients, with half the information questionnaire for patients and patients with disease information from the patient＇s case, do the single factor analysis of the above factors collection, analysis of significant factors of multi factor and the factor analysis of the light, that the risk factors of depression in elderly patients after stroke. Result Age greater than 65 years old, female, the course of more than 1 years, the lesion in the cerebellum, brainstem, frontal lobe in three, at high NDS as significant factors of depression in elderly patients after stroke ; gender, age, lesion location, in patients with NDS is a risk factor for depression in elderly patients after stroke. Conclusion Female, age is too high, the cerebellum, brainstem lesions, frontal lobe in three, NDS higher than the normal level are independent risk factors of depression in elderly patients after stroke, for this part of the clinical medical staff should be given special attention and take care of additional, in order to reduce the incidence of post stroke depression
