摘　　要：目的分析胃癌患者住院费用的构成特点及影响因素,为医疗机构及卫生行政主管部门管理决策提供参考依据。方法收集2010年1月-2014年12月胃癌患者住院费用清单,采用灰色关联分析、环比分析等方法,分析手术组与非手术组诊疗费用的构成和影响因素。结果单因素分析结果显示;性别、年龄、患病时间、患者来源、付费方式,与住院费用无统计学意义(P>0.5),住院天数、手术方式与住院费用有统计学意义(P<0.01)。总费用与各项费用关联序,无论手术组和非手术组药费排第1位。结论近年来某院胃癌患者人均住院费用增长速度高于同期本区居民收入的增长速度。加强对住院医疗费用构成及影响因素的管控,全面落实医保、农合等政策对于解决某地区重大疾病患者较重的经济负担具有重要作用。Objective To analyze the features of hospitalization expense pattern and influencing factors of patients with gastric carcinoma and propose proper measures to control hospitalization expenses, to provide reference basis for decision and management made by medical institutions and main administration departments of health and hospitals. Methods Collect the hospitalization expense lists of patients with gastric carcinoma from Jan. 2010 to Dec.2014, and adopt grey correlation and link relative ratio analysis methods to analyze the influencing factors of hospitalization expenses of surgical and non-surgical patients. Results Single factor analysis showed that gender,age, sick time, patient source and payment method have no statistical significance（P〉0.5）, and length of stay and hospitalization expenses have statistical significance（P〈0.01）. Grey correlation analysis on various expenses showed that medicine treatment of both operation group and non-operation group ranked the first in correlation order of total expenses and general expenses. Conclusions The increase rate of hospitalization expense per capita of 2010-2014 patients with gastric carcinoma is much higher than that of income of urban residents. The main influencing factors of hospitalization expenses are operation or not, operation methods, length of stay and expenses for medicine. Strengthening the control of hospitalization expenses, fully implementing health insurance and rural cooperative medical and other relevant policies play an important role in solving the problem of patients both suffering major diseases and heavy economic burden in a certain region.
