摘 要:目的观察Sirius与自动角膜曲率仪测量飞秒激光-准分子激光原位角膜磨镶术(FS-LASIK)前后患者角膜曲率的差异及测量结果的一致性,为Sirius在眼科临床的应用提供参考依据。方法 94例(188眼)行FS-LASIK术前检查患者,分别用Sirius、自动角膜曲率仪测量平坦子午线角膜曲率K1、陡峭子午线角膜曲率K2及平均角膜曲率Km,其中Km=(K1+K2)/2。FS-LASIK术后随访1周,随机选取20例(40眼),用Sirius、自动角膜曲率仪测量术后K1、K2、Km,采用配对样本t检验进行统计学分析;并采用Bland-Altman分析评价2种仪器测量角膜曲率的一致性。结果 FS-LASIK术前,Sirius与自动角膜曲率仪测量K1值差异无统计学意义(P>0.05),Sirius测量K2、Km值均大于自动角膜曲率仪(P<0.01)。FS-LASIK术后1周Sirius与自动角膜曲率仪测量K1、Km值差异均无统计学意义(P>0.05),Sirius测量K2值大于自动角膜曲率仪(P<0.05)。FS-LASIK术前,Sirius与自动角膜曲率仪测得K1、K2、Km值差值的均数接近差值为0的基线,95%一致性区间窄,FS-LASIK术后测量K1、K2、Km值差值的均数较术前增大,一致性区间增宽。结论在测量近视患者FS-LASIK术前角膜曲率时,Sirius与自动角膜曲率仪有较好的一致性,但术后1周一致性差,临床上是否可以相互取代需术后长期、大样本观察。Objective To evaluate the difference and consistency of Sirius and auto-keratometer in measuring corneal curvature in patients with myopia before and after femtosecond laser-laser in situ keratomileusis(FS-LASIK) , and to provide a reference for the application of Sirius in the field of ophthalmology. Methods Ninety-four patients( 188 eyes) who underwent FS-LASIK preoperative examination were enrolled in this study. The flat meridian corneal curvature K1, steep meridian corneal curvature K2 and average corneal curvature Km[ Km = ( K1 + K2)/2) ] were measured by Sirius and auto-keratometer respec- tively. The patients were followed up for one week after FS-LASIK. Twenty patients (40 eyes) were selected randomly and their K1, K2, and Km were measured by Sirius and auto-keratometer after FS-LASIK. A paired t test was used to analyze the resuhs. The consistency of the two devices in measuring corneal curvature ,sas analyzed by using Bland-Airman. Results Before FS- LASIK,the K1 value measured by the two instruments showed no significant difference(P 〉0.05) ;but the K2 and Km values which measured by Sirius were significantly higher than those by auto-keratometer( P 〈 0.01 ). After FS-LASIK, the K1 and Km values measured by Sirius and auto-keratometer showed no significant difference (P 〉 0. 05 ) ; but the K2 value measured by Sirius was significantly higher than that by auto-keratometer (P 〈 0.05 ). Before FS-LASIK, the difference of the K1, K2 and Km between Sirius and auto-keratometer measurements were closer to the line of 0. The 95% limits of consistency were nar- row. Compared with before FS-LASIK, the difference of the K1, K2 and Kin between Sirius and auto-keratometer measurements after FS-LASIK were higher and the 95% limits of consistency were broader. Conclusions Good consistency is exhibited in corneal curvature of myopia patients between Sirius and auto-keratometer before FS-LASIK.
