摘  要:目的分析不同颊舌径的下颌第一磨牙种植冠在不同载荷下,种植体周骨质的受力情况,为临床提供种植冠颊舌径的最适合范围。方法用三维有限元法对不同颊舌径的下颌第一磨牙种植冠,施加垂直均布、垂直集中、45°斜向的载荷,对种植体骨界面上的应力进行分析。结果不同方式的加载力,种植体周围骨质承受应力大小比较:45°斜向>垂直集中>垂直均布;随着颊舌径减小,种植体周围骨质受到的von Mises应力值也相应减小。将颊舌径减小成为原来颊舌径的60%时,种植体及周围骨质受到的最大von Mises应力值最小;但是应力集中的部位没有改变,均是在种植体颈部周围的皮质骨内。结论下颌第一磨牙种植体冠的颊舌减径能起到降低种植体及其周围骨质应力集中的受力情况,所以在种植修复时,科学合理的优化义齿的颊舌径设计,可以有效的保护种植体骨界面的形成,提高种植修复的成功率。Objective To analyze the stress distribution on the implant-bone interface of the mandibular first molar implant crown with varying buccolingual diameter,for getting the most suitable diameter range of the implant crown for clinical. Methods Analyzing the stress on the implant-bone interface of the different mandibular first molar implant crown with five different buccolingual diameters with the Three-dimensional finite element. which applied in this study was a vertical concentrate loading,a vertical uniformly distributed loading and a 45 degree slope loading. Results Under the different kinds of loading, the stress on the implant-bone interface was: 45 degree oblique vertical concentrated vertical uniformly distributed. The Von Mises stress on the implant-bone surface decreasing with the reducing of the buccolingual diameter, and when the diameter became 60of the original, the maxium Von Mises was the minimal. But the position where the stress concentrating on was in the cortical bone around the implant neck which was never changed.Conclusion The reducing of the buccolingual diameter of the mandibular first molar impalnt crown can lead to the decressing of the stress concentrated on the implant-bone interface. So the optimization of the reasonable and scientific buccolingual diameter design can protect the formation of the implant-bone interface effectively, and improve the success rate of the implant restoration.
