改良唇挡在单侧完全性唇腭裂患者上颌不对称扩弓治疗的临床疗效研究

摘　　要：目的观察改良唇挡对单侧完全性唇腭裂患者上颌不对称扩弓的临床治疗效果。方法选择上颌牙弓狭窄、磨牙区宽度正常的单侧完全性唇腭裂患者22例,应用改良唇挡悬吊牵引裂隙侧牙齿使其颊向移动,从而达到扩弓目的,在模型上测量牵引前、后上颌尖牙、双尖牙、磨牙间的牙弓宽度,对测量结果进行统计学分析。结果 22例患者经9~11个月的治疗,裂侧上颌尖牙、第一双尖牙、第二双尖牙颊向移动分别为9.30mm 6.33mm和3.50mm,磨牙间的宽度基本没有变化。结论改良唇挡可有效实现对唇腭裂患者上颌牙弓的不对称扩弓。Objective To investigate the effect of the modified lip bumper on asymmetric expansion in patients with unilateral complete cleft clip and palate. Methods Twenty-two patients with unilateral complete cleft clip and palate, having maxillary intraversion except molars area, were enrolled in this study. The modified lip bumpers were used to buccally tract the teeth in the cleft side. And then models were made before and after the traction. And the arch width between the maxillary canines, the premolars and the first molars were measured and statistically analyzed.Results After the patients were treated for 9 to 11 months, the canines and the premolars in the crossbite side was respectively buccally locatted by 9.30 mm, 6.33 mm and 3.50 mm, but the first molars nearly maintained immobilization.Conclusion The modified lip bumper is an effective method in expansion in patients with unilateral complete cleft clip and palate.
