结肠运输试验联合排粪造影在慢性便秘进一步诊断中的应用价值

摘　　要：为评估结肠运输试验联合排粪造影检查对慢性便秘进一步诊断的价值，对140例临床诊断为慢性便秘的患者采用数字胃肠机行结肠运输试验和排粪造影检查（检查前给予结肠灌洗）。结果显示，1）结肠运输试验结果：结肠运输正常35例，运输异常105例。2）动态排粪造影结果：正常24例，异常116例。116例异常者中，直肠前突29例，直肠前突＋会阴下降30例，直肠前突＋直肠黏膜内脱垂16例，直肠黏膜内脱垂＋直肠内套叠17例，直肠前突十耻骨直肠肌肥厚4例，乙状结肠冗长＋直肠前突＋直肠黏膜内脱垂＋直肠内套叠＋（盆底）肠疝＋会阴下降6例，会阴下降5例，盆底痉挛综合征4例，内脏下垂＋会阴下降5例。3）综合结果：81例同时有结肠运输异常和出口梗阻。结果表明，结肠运输试验联合排粪造影检查在慢性便秘患者进一步诊断中应用价值高，可为临床治疗提供可靠的依据。In order to estimate the application value of colon transport test combined with defecography to further diagnose chronic constipation, authors again inspected 140 patients who were clinically diagnosed as chronic constipation by using colon transport test（performed by digital gastro-enterological apparatus） and defecography（first colon lavage before examination） . As results, 1）colon transport test showed：35 eases being normal, 105 being abnormal;2）dynamic defecography showed：24 cases being normal, 116 being abnormal amorig whom 29 were of rectocele,30 of rectocele ＋perineum descended, 16 of rectocele ＋ internal rectal mucosa proIapsed, 17 of internal rectal mucosa prolapsed ＋ rectal inner-intussusception,4 of rectocele ＋puborectal musle pachynsis, 6 of sigmoid redundancy ＋ rectocele ＋ internal rectal mucosa pro-lapsed＋rectal inner-intussusception ＋ （pelvic） enteric hernia ＋ perineum descended, 5 of perineum descended,4 of spastic pelvic floor syndrome, 5 of viscus descensus ＋ perineum descended; 3）combined results：81 cases had either abnormal colon transport and outlet obstruction at same time.Resuhs show that above combined inspection for further diagnosing chronic constipation has high application value, can pro-vide reliable basis to clinical treatment.
