摘 要:目的:探讨肝脏损伤患者IL-2、IL-6水平变化及对乙肝疫苗免疫应答的影响及其机制。方法:选取2013年6月~2014年10月收治的肝损伤患者32例(肝损伤组),选取同期体检健康无肝损伤者20例作为对照组,采用化学发光法测定两组HBsA抗体的滴度,采用流式细胞仪测定血液中的淋巴细胞亚群,采用ELISA的方法测定血清中IL-2和IL-6的含量。结果:肝损伤组的抗HBsAb的单抗的滴度明显低于对照组(P〈0.05);肝损伤组的细胞免疫指标CD3+、CD4+、CD19+及NK细胞的含量明显低于对照组,CD8+细胞的含量高于对照组,CD4/CD8的比值低于对照组(P〈0.05);肝损伤组的血清中IL-2含量明显低于对照组,IL-6含量明显高于对照组(均P〈0.05)。结论:肝损伤可引起的体内IL-2、IL-66变化,可降低乙肝疫苗的免疫应答效果,导致免疫系统紊乱。Objective: To explore the effect of IL-2 and IL-6 on immune response of hepatitis B vaccine in patients with hepatic injury. Methods: A total of 32 cases with hepatic injury admitted from June 2013 to October 2014 were selected, and 20 healthy persons without injury were selected as control group. Chemiluminescence assay was used to detect titer of HBsA anti- body, flow cytometry was used to detect lymphocyte subsets, and ELISA was used to detect contect of IL-2 and IL-6 in serum. Results: Titer of monoclonal antibody of anti-H13sAb in injury group was significantly lower than that of control group (P〈 0.05). Contents of CD3+, CD4+, CD19+ and NK were significantly lower than these of control group (P〈0.05). IL-2 level of injury group was significantly lower, while IL-6 level was significantly higher (P〈0.05). Conclusions: Hepatic injury can cause change on IL-2 and IL-6. It can weaken immune response of hepatitis 13 vaccine, and lead to disorder of immune system.
