摘 要:[目的]探讨超声弹性成像应变率比值在鉴别甲状腺良恶性结节中的应用价值。[方法]对70例甲状腺结节患者检出的87个结节进行超声弹性成像检查,并测量弹性成像应变率比值,绘制ROC曲线图,确定区分甲状腺良恶性结节应变率比值的临界点。[结果]甲状腺恶性结节SR比值平均为5.30±1.26,良性结节SR比值平均为2.79±1.19,两组间差异具有统计学意义(P<0.05)。甲状腺结节弹性应变率比值的ROC曲线下面积为0.936,选择约登指数最大切点0.71为临界点,对应的SR比值为3.63,其对应的灵敏度为97.3%,特异性为73.5%。[结论]超声弹性成像应变率比值能客观地反映甲状腺结节的相对硬度,有助于甲状腺良恶性结节的鉴别诊断,在临床诊断中具有较高的应用价值。[Purpose] To investigate the evaluation of strain ratio for ultrasonic elastography in differential diagnosis of benign and malignant thyroid nodules. [Methods] Ultrasound elastography detection was performed for 70 patients with 87 thyroid masses. The strain ratio of each lesion was calculated. According to the ROC curve,the critical point of the strain ratio of different lesions was achieved. [Results] The average of strain ratio of malignant masses was 5.30 ±1.26,while the average of strain ratio of benign masses was 2.79 ±1.19.There were statistical differences between them(P〉0.05). According to the ROC curve,the area under the curve was 0.936.It was concluded that the critical point of strain ratio was 3.63,and the corresponding sensitivity was 97.3%,the specificity was 73.5%. [Conclusion] The strain ratio for ultrasonic elastography is useful in the differential diagnosis of benign and malignant thyroid nodules.
