摘 要:目的了解杭州地区新生儿及婴儿TORCH感染状况及临床特点。方法应用ELISA法检测1 220例新生儿和婴儿血清中的TORCH特异性抗体,并进行相关分析。结果受检患儿中TORCH-Ig M阳性率为2.3%,其中以CMV感染为主,婴儿期CMV-Ig M阳性率达9.2%;患儿中TORCH-Ig G抗体阳性率为98.3%,其中单一病原体感染者Ig G抗体检出构成比为9.8%,病原体以HSVⅠ最常见;多重感染中,3种病原体感染者最多,Ig G抗体检出构成比为56.2%,病原体组合模式以HSVⅠ、RV、CMV为最多,构成比为53.3%;分组比较新生儿及婴儿Ig G抗体检测均以HSVⅠ-Ig G抗体阳性率最高,分别为94.0%、68.9%,其次是CMV-Ig G,分别为90.2%、64.7%;CMV-Ig M阳性患儿临床疾病类型以肺炎和支气管炎为主。结论 TORCH感染在新生儿及婴儿期发生率高,婴儿期需警惕CMV感染,应及早筛查,降低出生缺陷和死亡率。Objective To investigate the condition and clinical feature of TORCH infection in neonates and infants in Hangzhou. Methods TORCH specific antibodies in serum samples from 1 220 neonates and infants were tested by the enzyme linked immunosorbent assay( ELISA),and did related analysis. Results Among the infected neonates and infants,positive rate of TORCH- Ig M was 2. 3%,and the most common pathogenic virus was CMV,positive rate of CMV- Ig M in infants was9. 2%; positive rate of TORCH- Ig G in patients was 98. 3%. Ig G positive rate of single pathogenic virus detected was 9. 8%,HSV Ⅰwas the most common pathogenic virus. In multiple infections,the infection of the 3 pathogens was the most,and the proportion of Ig G antibody was 56. 2%. The most common pathogen combination model were HSV Ⅰ,RV,CMV,accounting for 53. 3%. Comparing Ig G antibody detection in neonates and infants in different groups,the HSV Ⅰ- Ig G positive rate were highest in both neonates and infants,which were 94. 0% and 68. 9%,respectively. CMV- Ig G was the second pathogenic virus in neonates and infants,the positive rate was 90. 2% and 64. 7%,respectively. Pneumonia and bronchitis was the main clinical symptoms in these neonates whose serum CMV- Ig M were positive. Conclusion The infection rate of TORCH are very high in neonates and infants,we should be wary of CMV infection in infants period. Screening tests for TORCH infection should be done in neonatal period to reduce birth defects and neonate mortality.
