牙周基础治疗对冠心病伴牙周炎患者血脂和血清CRP的影响

摘　　要：目的探讨牙周基础治疗对冠心病伴牙周炎患者的血脂和血清C反应蛋白(C-reactive protein,CRP)的影响。方法选择38例冠心病伴牙周炎患者进行牙周基础治疗。比较患者治疗前后的牙周临床指标(探诊深度、龈沟出血指数、菌斑指数、附着丧失)、血脂以及血清CRP水平。结果牙周基础治疗后,患者的牙周临床指标得到有效控制(P<0.05)。治疗前与治疗后3个月血脂与血清CRP情况如下:总胆固醇由(5.73±1.39)mmol/L降至(4.25±1.24)mmol/L(t=6.25)、甘油三酯由(2.05±0.96)mmol/L降至(1.72±0.51)mmol/L(t=2.39)、低密度脂蛋白胆固醇由(3.11±0.98)mmol/L降至(2.62±0.85)mmol/L(t=2.974);而高密度脂蛋白胆固醇则由(1.03±0.74)mmol/L升高到(1.46±0.82)mmol/L,差异均具有统计学意义(P<0.05);治疗前、后血清CRP水平分别为(4.46±0.91)mg/m L、(14.38±1.16)mg/m L,两者比较,差异有统计学意义(t=52.979,P<0.01)。结论有效的牙周基础治疗能够改善冠心病伴牙周炎患者的血脂及血清CRP水平,有利于冠心病的预防和治疗,具有重要的临床意义。Objective To discuss the effect of initial periodontal treatment on lipids and serum C-reactive protein（ CRP） of patients with coronary artery disease and periodontitis. Methods Thirty-eight cases of patients with coronary heart disease and periodontitis were selected, and the patients were treated by initial treatment of periodontitis. the periodontal probing depth（ PD）, bleeding index（ SBI）, plaque index（ PLI）, attachment loss（ AL）, the levels of lipids and CRP of patients were observed and compared before and after the treatments. Results After initial treatment, periodontal disease were effectively controlled. Changes of serum liqids and CRP were as follow. Total cholesterol fell from（ 5. 73 ± 1. 39） mmol / L to（ 4. 25 ± 1. 24） mmol / L, Triglyceride fell from（ 2. 05 ± 0. 96） mmol / l to（ 1. 72 ±0. 51） mmol / L, low density lipoprotein cholesterol fell from（ 3. 11 ± 0. 98） mmol / l to（ 2. 62 ± 0. 85） mmol / L and high density lipoprotein cholesterol increased from（ 1. 03 ± 0. 74） mmol / L to（ 1. 46 ± 0. 82） mmol / L. The results were statistically significant（ P〈0. 05）. The level of serum CRP was significantly decreased from（ 14. 38 ± 1. 16） mg / m L before treatment to（ 4. 46 ± 0. 91） mg / m L after treatment（ P〈0. 01）. Conclusion Effective periodontal therapy can improve the level of serum lipids and serum CRP in patients of coronary heart disease with periodontitis, and it is beneficial for the prevention and treatment of coronary heart disease..
