女性安氏Ⅱ类错伴颞下颌关节紊乱病患者髁突位置分析

摘　　要：目的探讨女性安氏Ⅱ类错伴颞下颌关节紊乱病(temporomandibular disorders,TMD)患者的髁突位置。方法收集26例女性安氏Ⅱ类错伴有临床症状的TMD患者,利用锥形束CT(cone beam computed temography,CBCT)对患者的双侧颞下颌关节进行扫描,测量颞下颌关节前、上、后、外、内间隙,分析髁突位置。结果26例女性安氏Ⅱ类错伴TMD患者颞下颌关节各间隙间左右侧之间均无统计学意义(P>0.05),两侧颞下颌关节间隙合并统计,前、上、后、外、内间隙分别为(2.343±0.827)mm,(2.138±0.807)mm,(1.959±0.757)mm,(1.764±0.609)mm,(2.103±0.663)mm;通过计算ln(P/A)值和线性比率(linear ratio,LR)得出髁突后移位分别占50%、48%。结论女性安氏Ⅱ类错畸形患者髁突在关节窝内处于后位可能是导致TMD的原因之一。Objective To investigate the condylar position of women patients with Angle ClassⅡ malocclusion and temporomandibular disorders（ TMD）. Methods A total of 26 women patients with Angle ClassⅡ malocclusion and temporomandibular disorders（ TMD） were selected. Their bilateral temporomandibular joint were scanned by cone beam CT（ CBCT） for linear measurements of anterior interspace,upper interspace, rear inerspace, outer interspace and inner interspace of TMD to evaluate their condylar position. Results There was no significant difference between left and right temporomandibular joint spaces, so the left and right temporomandibular joint spaces were merged for statistics. The mean anterior interspace, upper interspace, rear inerspace, outer interspace and inner interspace were（ 2. 343 ± 0. 827） mm,（ 2. 138 ± 0. 807） mm,（ 1. 959 ± 0. 757） mm,（ 1. 764 ± 0. 609） mm,（ 2. 103 ± 0. 663） mm respectively. With the method of calculating the ln（ P / A） and linear ratio（ LR）, condylar position move backward for fifty percentage and fortyeight percentage respectively. Conclusion In women patients with Angle ClassⅡ malocclusion, posterior condylar position may play an important role in TMD.
