摘 要:目的:从文献计量学角度对国内外脑室内或鞘内注射抗菌药物治疗中枢神经系统(CNS)感染方面相关文献进行分析,旨在为抗感染工作领域研究者提供参考。方法:检索Pub Med、SCI、Embase、中国知网等数据库,检索时间为建库至2014年3月,采用文献计量学方法,应用End Note 6.0软件进行分类整理,对纳入文献的年代、通讯作者、文章类型、被引文频次等进行统计分析。并对CNS感染的原因、主要疾病、症状、治疗现状以及脑室内或鞘内注射抗菌药物特点进行分析讨论。结果:共检索文献1737篇,符合纳入标准540篇,分布在355种期刊上,按发文量《Journal of Antimicrobial Chemotherapy》排名第一;文献量总体呈现上升趋势,主要文章类型为病例报告和综述均为183篇;单篇文章被引文频次最高为560次,发文数排名前三位的国家依次为美国、英国、日本。结论:脑室内或鞘内注射抗菌药治疗CNS感染方法越来越受到关注,因其可避免血脑屏障影响,在脑室内或鞘内达到更高的药物浓度,效果比传统方法更佳,但目前治疗效果主要体现在病例报告中,还需要高质量的随机对照临床试验确证其治疗效果。Objective: To investigate the treatment with intraventricular or intrathecal injection of antibiotics for CNS infection based on a bibliometric approach and provide reference for anti-infection researchers. Methods: We searched PubMed, SCI, Embase, CNKI database, etc. EndNote 6.0 software was used to sort these articles from the publication year, corresponding author, literature type, cited frequency, etc. Cause of CNS infection, main diseases, symptoms, route of infection, treatment status, the difference between traditional treatment methods and intraventricular or intrathecal injection of antibiotics were analyzed. Results: A total of 1737 papers were collected, 540 papers met the inclusion criteria, distributed over 355 kinds of journals. Journal of Antimicrobial Chemotherapy was the top in respect of the published paper numbers. The numbers of literature showed an overall upward trend, meanwhile the case report and review were the main type of literature. The highest cited frequency of paper was 560. The top nation with published paper numbers was the United States, following by England and Japan. Conclusion: The treatment by intraventricular or intrathecal injection of antibiotics in CNS infection with higher concentration and good efficacy attracts more attention for its avoiding the blood-brain barrier impact. While the effect was only reported by case report, high quality randomized controlled clinical trials are needed to confirm its therapeutic effect.
