眼前节OCT对深板层角膜内皮移植术后植片形态学特征的评价(英文)

摘　　要：【目的】探讨眼前节OCT对深板层角膜内皮移植术后植片形态学改变的作用和临床应用价值。【方法】对26例大泡性角膜病变患者行深板层角膜内皮移植术,应用眼前节光学相干断层成像术（眼前节OCT）分别于术后1周和1、3、6、12月对植片和植床厚度、上皮下大泡形成、内皮植片皱褶、内皮植片脱离等形态学特征进行动态观察和定量分析。【结果】术后1周角膜内皮植片平均厚度为（173±56）μm,与术后1、3、6、12月内皮厚度相比无明显统计学差异（P=0.21）;术后1周角膜植床平均厚度为（412±57）μm,与术后1、3、6、12月植床厚度相比有明显统计学差异（P=0.033）;术后1周9眼（34.62%）出现边缘区域植片-植床裂隙,术后6月全部愈合消失,7眼（26.92%）出现上皮下大泡,并于术后1~3月消失;术后1周6眼（23.08%）出现内皮植片皱褶,术后12月时内皮皱褶消失;术后1周2眼（7.69%）出现局限性内皮植片脱离并伴有相应区域的上皮下大泡形成,分别于术后1月和3月时愈合。【结论】眼前节OCT对深板层角膜内皮移植术后植片形态学特征的评价安全、有效,并能提供动态观察和定量分析,对深板层角膜内皮移植术后疗效评价具有临床应用价值。【Objective】 To evaluate donor graft morphology after deep lamellar endothelial keratoplasty（DLEK） with anterior segment optical coherence tomography（OCT）. 【Methods】 A retrospective case series. Twenty-six endothelial grafts after DLEK were analyzed using anterior segment OCT at 1 week and 1, 3, 6, and 12 months postoperatively. The central thickness of donor graft and recipient bed, gaps between graft and donor, epithelial bullae and the morphology of endothelial graft were observed. 【Results】 The average central thickness of donor graft was（173 ± 56） μm on 1 week after surgery, there were no significant difference compared to the data at 1,3,6, and 12 months postoperatively（P = 0.21）. The average central thickness of recipient bed was（412 ± 57） μm,there were significant difference compared to the data at 1, 3, 6, and 12 months postoperatively（P = 0.033）. 1 week after surgery,the gaps at the border of graft endothelium and recipient bed existed in 9 eyes（34.62%）, and the epithelial bullae above the gap existed in 7 eyes（26.92%）. The folds of graft endothelium occurred in 6 eyes（23.08%） at 1 week postoperatively and disappeared in12 months after surgery. There were 2 eyes（7.69%） showed localized endothelial graft detachment with epithelial bullae, that healed at 1 months and 3 months after surgery respectively. 【Conclusion】 Anterior segment OCT could provide detailed information of graft and that is safe and valuable for evaluating graft morphology after DLEK.
