双能量CTA在诊断颈动脉狭窄和斑块中的临床应用价值

摘  要:目的探讨双能量CTA在诊断颈动脉狭窄和斑块中的临床应用价值。方法应用双源CT双能量CTA检查80例缺血性脑梗死患者颈动脉,分析双能量CTA自动减影去骨技术对颈动脉的显示,并评价图像质量。其中有30例患者在两周内选择了DSA检查,并与其进行比较。同时对检测患者颈动脉粥样硬化斑块的分布、类型及颈动脉狭窄程度进行比较。结果 80例患者颈动脉双能量自动减影去骨后,颈动脉血管总体满意显示率为86.5%,其中颈内动脉颅外段血管满意显示率最高(97.3%)。30例阳性患者颈动脉双能量自动去骨后与DSA对照,狭窄吻合率基本一致,尤其中度狭窄以上达到90%以上。80例患者中72例检出斑块,其中颈内动脉起始部斑块检出率最高,软斑块多位于CCA段(114/228),硬斑块多位于ICA-I段(97/207),混合斑块多分布在ICA-E段(44/41),各段左右侧斑块分布无明显差异。结论双源CT双能量颈动脉成像是一种简便、快速、无创性检查方法,通过分析颈动脉斑块及狭窄程度为脑血管事件的防治提供了较高的参考依据。Objective To explore the clinical application value of dual energy CTA in the diagnosis of carotid artery stenosis and plaque. Methods A retrospective analysis in our hospital application of dual energy CT carotid angiography in 80 patients, Analysis of carotid artery in patients with dual energy CT automatic bone removal technology on carotid artery, and evaluate the image quality. The positive results in 30 patients with DSA examination in two weeks, and with the DSA results as the gold standard of comparison, dual energy CT automatic subtraction bone technology in clinical value in the diagnosis of carotid artery stenosis. Simultaneous detection of patients with carotid atherosclerosis and vascular stenosis, and compared the distribution of plaque, the type and extent of carotid artery stenosis. Results In 80 cases of carotid artery in patients with dual energy automatic bone, carotid artery overall satisfaction was 86.5%, the extracranial internal carotid artery(ICA-E) show the rate was the highest(97.3%).30 cases patients with dual energy automatic bone and DSA control, narrow coincidence rate is basically the same, especially in severe stenosis and occlusion of the coincidence rate reached above 95%. In 80 patients, 72 cases were detected plaque, soft plaque located in the CCA segment(114/228), hard plaque located in the ICA-I segment(97/207), mixed plaque distribution in the ICA-I section of the lowest(23/108), there was no difference in CCA segment and ICA-E segment distribution(44/41), the left and right side plaque distribution no significant difference. Conclusion Dual source CT dual energy carotid artery imaging is a simple, rapid, noninvasive examination method, based on the analysis of carotid plaques and the stenosis degree of and provides a high reference for the prevention and treatment of cerebrovascular events.
