局部缓释唑来膦酸对家兔下颌骨骨折愈合的影响

摘　　要：目的探讨局部应用缓释唑来膦酸对家兔下颌骨骨折愈合的影响。方法 24只成年新西兰家兔,随机分为对照组和实验组各12只,选择左侧下颌骨体制备线形骨折模型,应用钛板内固定后,对照组局部覆盖生理盐水缓释膜,实验组局部覆盖唑来膦酸缓释膜。分别于术后4、8、12周各处死4只家兔,截取2组骨折区骨块行组织病理学检查观察骨折区组织改变情况,采用免疫组织化学法检测骨形态发生蛋白-2表达情况。结果实验组骨折处新骨形成较对照组快;术后4、8周实验组骨折区骨形态发生蛋白-2表达阳性率(76.7%、53.3%)高于对照组(55.0%、33.3%)(P<0.05);术后12周实验组骨折区骨形态发生蛋白-2表达阳性率(20.0%)与对照组(13.3%)比较差异无统计学意义(P>0.05)。结论局部应用缓释唑来膦酸可促进兔下颌骨骨折愈合,缩短骨折愈合时间。Objective To explore the effect of local slow-release application of zoledronic acid on the rabbits mandibular fracture healing. Methods Twenty-four adult New Zealand rabbits were randomly divided into control group and experimental group, with 12 rabbits in each group. The left mandibular body was selected to make linear fracture model. After internal fixation with titanium plate, control group was locally covered with saline-release film and experimental group was locally covered with zoledronic acid-release film. After 4, 8 and 12 weeks of operation, four rabbits were sacrificed to obtain bone from the fracture zone. The histopathological method was used to observe the tissue changing in bone fracture zone, and immunohistochemistry was adopted to detect the expression of bone morphogenetic protein-2 （BMP-2）. Results New bone formation was sooner in experimental group than control group, and BMP-2 positive expression rates after 4 and 8 weeks of operation were higher in experimental group （76. 7%, 53. 3%） than those in control group （55. 00%, 33. 3%） （P〈 0.05）. There was no significant difference in BMP-2 positive expression rate between experimental group （20.0 %） and control group （13.3 %） after 12 weeks of operation （P〉0.05）. Conclusion Local slow-release application of zoledronic acid can accelerate mandibular fracture healing and shorten the fracture healing time in rabbits.
