不同糖耐量状态下非酒精性脂肪肝病患者血清Nesfatin-1水平的比较及相关因素的研究

摘　　要：目的研究不同糖耐量状态下非酒精性脂肪肝病(NAFLD)患者血清Nesfatin-1水平及对相关因素进行分析。方法选取NAFLD患者126例,按照口服葡萄糖耐量(OGTT)结果分为糖耐量正常组(NGT组)、糖耐量减低组(IGT组)及2型糖尿病组(T2DM组)。比较各组空腹状态下Nesfatin-1、体质指数(BMI)、空腹血糖(FBG)、空腹血浆胰岛素(FIN)、Hb A1C、TC、TG、LDL-C、HDL-C、HOMA-IR等指标变化。结果 NAFLD患者从NGT到IGT,再到T2DM,血清Nesfatin-1水平逐渐升高,差异有统计学意义(P<0.05)。Nesfatin-1与BMI呈负相关,TG是Nesfatin-1的独立影响因素(β=-0.328,P<0.05)。结论 NAFLD患者在不同糖耐量状态下,血清Nesfatin-1和糖脂代谢均密切相关,Nesfatin-1可能在NAFLD患者从NGT发展成为T2DM的过程中具有重要的作用。【Objective】 To observe the level of serum Nesfatin-1 and the related factors in patients of nonalcoholic fatty liver disease（NAFLD） with different glucose tolerance. 【Methods】 A total of 126 patients with NAFLD were divided into three groups： normal glucose tolerance（NGT） group, abnormal glucose tolerance（IGT） group and type 2 diabetes mellitus（T2DM） group. They were all selected to measure Nesfatin-1, BMI,FBG, fasting plasma insulin level（FIN）, Hb A1 C, TC, TG, LDL-C, HDL-C and HOMA-IR. 【Results】 During the course of nonalcoholic fatty liver disease, the level of serum Nesfatin-1 gradually increased from NGT,IGT to T2 DM, the differences were statistically significant（P〈0.05）. Nesfatin-1 was negatively correlated with BMI. TG level was an independent relevant factor of Nesfatin-1（β =-0.328, P〈0.05）. 【Conclusions】 In the condition of different glucose tolerance with NAFLD, serum Nesfatin-1 is closely related with glucolipid metabolism. Nesfatin-1 might play an important role in the occurrence and development of T2 DM with nonalcoholic fatty liver disease.
