肠道淋巴管瘤的临床病理分析

摘　　要：目的探讨肠道淋巴管瘤的临床病理特征,对该类肿瘤的临床诊断和治疗提供更多的研究资料。方法复习了12例肠道淋巴管瘤的临床资料,大体形态、并对其组织学切片与免疫组化标记进行检查,同时结合已有文献总结分析该类肿瘤的临床病理特征。结果本文12例病例中,4例为小儿患者,8例为成人患者。小儿患者临床首发症状多为持续的不同程度的腹痛,伴或不伴恶心、呕吐等消化道症状,临床诊断为肠梗阻或肿瘤;而成人患者多为偶然发现,临床诊断多为黄色瘤或粘膜隆起性质待定。病理学检查显示粘膜层、粘膜下层或浆膜层见多个明显扩张,大小不均、囊壁厚薄不一的淋巴管,内衬单层扁平上皮细胞,囊腔内可见伊红色的淋巴管液,并可见多少不等的淋巴细胞。免疫组化标记肿瘤细胞均D2-40,CD31阳性,部分CD34阳性,Calratinin,WT-1阴性。结论肠道淋巴管瘤临床十分少见,手术切除一般预后良好。Objective To study the clinicalpathological features of intestinal lymphangioma, provide more data for the diagnosis and clinical treatment. Methods Review the clinical data of 12 with intestinal lymphangioma cases, general form,also all of the biopsy slides and immunohistochemical markers, combined with the existing literature,in order to summarize clinical pathologic features of this tumor.Results In this paper,, 4 of 12 cases were pediatric patients, 8 of 12 cases were adult patients. Pediatric patients with clinical symptoms start to varying degrees of abdominal pain, with or without associated with gastrointestinal symptoms such as nausea, vomiting, and the clinical diagnosis could be intestinal obstruction or tumor; And adult patients were found by chance usually, clinical diagnosis could be xanthoma or mucosal patch to be determined. Pathological examination showed mucosa, submucosa or serosal of intesine would be discovered multiple expansion, different sizes, different thickness of capsule wall of lymphatic vessels, lined with simple squamous epithelium cells, can be seen in the lumen of the red lymphatic fluid, with varying amounts of lymphocytes. Immunohistochemical markers D2- 40, CD31 always would be positive, part of CD34 positive,Calratinin and WT 1 showed negetive. Conclusion Lymphangioma of intestinal isvery rarein clinical work, and surgical removal will haveagood prognosis.
